Description of Occurrence:

At_1745 hours an attempt was made to conduct a quarterly surveillance.

test on the Reactor Building cooling units,.per Operating Procedure 1104.33. Upon command to open, CV-3813 failed to clear its closed limit but CV-3815 responded properly. The valve was repaired and A0 report number 50-313/75-1 was transmitted. The alternate loop service i

water 1:,olation valves were stroke ' tested and CV-3812 responded properly,-but CV-3814 did not. Cause of. failure could not be immedia-tely determined.

Corrective Action:

The construction of these valves are butterfly type with a vertical stem of stainless steel with a stainless steel bushing on-each side of'the bdtterfly and there are no provisions for lubrication of the bushings.

1 Rue stem was polished to remove scratches and upset metal on its surface and the bushings were honed smooth. The bushings were lubricated with

- Fel-Pro N-1000, the valve was reassembled and verified operable.
