Appendix A NOTICE OF VIOLATION o
Toledo Edison Company Docket No. 50-346 Based on the inspection conducted on May 5, 16-18, 25, 26, 31; June 1, 2, and 20-22, 1977, it appears that certain of your activities were in noncompliance with NRC requirements as noted below.
Items 1 through 3 are deficiencies.
1.
Contrary to Section 6.5.1.6.c of the Tect.nical Specifications, l
the reques t for proposed licensee amendment dated May 20, 1977, was not reviewed by the Station Review Board prior to the request.
2.
Contrary to Section 6.5.1.8 of the Technical Sp'ecifications, the violation of Technical Specification described in reportable cucurrence NP-33-77-2 was not documented as having been reviewed by the Station Review Board.
3.
Contrary to the approved Quality Assurance Manual and Criterion V of 10 CFR 50, Appendix B, Administrative Procedure 1807.00 was not completely adhered to in the processing of deviation reports.
