Dear Mr. Kenna:
In item number 3 of the referenced inspection report, two previously reported items of noncompliance remain open.
In both cases, the Lynchburg A
Research Center comitted to have the required forms printed and in use by June 1, 1980.
We are continuing to experience difficulty in getting the forms back from the printer.
I, therefore, request that we be granted an extension of the completion date to July 1,1980.
In the interim, the Lynchburg Research Center is utilizing a type form containing all the information that will appear on the printed ones. The typed forms also provide for the reviewer's signature.
Your consideration in this matter is appreciated.
