Narrative

Report for LER 50-366/1980-053, Rev. 2 During normal plant operation, notification was received from i

the A/S, Bechtel Power Corporation, via telecopy that a design defect exists on Hatch 2 H2-02 analyzers as identified by the manufacturer, Comsip, Inc.

A sample pump was deadheaded during testing by the manufacturer and a pressure buildup was noted to have the potential of a diaphragm rupture.

Comsip reported Lnis event to the Region V NRC director under 10CFR21 requirements and listed llatch 2 as one of the plants affect-ed.

A diaphragm failure was experienced during a PCIS test as reported on ' LER 50-366/1980-065.

As an interim pro tec ti on I

scheme against diaphragm rupture, a standing order has been issued to trip the sample pump upon a Group II isolation.

Additionally, operators are to check that the pump suction and discharge valves are open before returning the pump to service.

A design change request and material requisition have been generated to install a bypass around the pump to recirculate the process gas in the isolated condition by l

7-31-60.

Also an interlock to trip the sample pump upon isolation of the PCIS valves will be implemented during the next refueling outage.
