  • ' NOTICE'0F VIOLATION Based on the results of the NRC inspection conducted May 16-20,1977, it appears that certain of your activities were not conducted in full compliance with your Reac, tor License or NRC requirements as indicated below:

Criterion V of Appendix B to 10 CFR 50 requires that activities affecting quality shall be prescribed by documented procedures and shall be accomplished in accordance with these procedures. This

. requirement is further amplified by Section 5 of the licensee's NRC approved Quality Assurance Pr.ogram.

Administrative Control Procedure #1005.01, Rev. 2, T/C #7, dated March 28,1976, Section 6.1.5.A.1 states that, " Control room 1.ogs and records are important for the information they provide for current operations and for the history of cperations. All log entries, data sheet entries and chart notes must be legible, complete and under-standable." Section 6.1.5.A.2 states, "The Supervisor of Operations is responsible for a review of all logs and records.

He will advise Shift Supervisors of any shortcomings of operator entries and will e

initiate measures to eliminate or minimize the shortcomings."

Contrary to the above, the Reactor Protection System Log for April 29, 1977 was not completed as required nor were any steps apparently taken to eliminate or minimize the shortcomings.

This is an infraction.

Technical Specification 6.8.1 requires that " written procedures shall be established, implemented and maintained covering... the applicable procedures recommended in Appendix A of Regulatory Guide 1.33; (and) surveillance and test activities of safety related equipment...."

The requirement to maintain such procedures is amplified in Section 5.6.2 of the Quality Assurance Manual which requires that " Applicable instructions, procedures and drawings shall be reviewed, and revised as necessary, following modifications to the plant."

l Contrary to this requirement, Procedure 1302.03 " Periodic Calibration of Incore Detectors," dated August 30,1974, a procedure necessary to the proper performance of the incore detectors, was not updated to reflect a modification to the detector circuits that was performed in l
