Gentlemen:
The subject inspection report identified one violation of our Technical Specifications and one violation of Criterion V of Appendix B to 10CFR50.
As applicable, our response to the violation includes:
(1) correc.tive steps which have been taken and results achieved; (2) corrective steps which will be taken to avoid further non-compliance; and (3) the date when full compliance will be achieved.
The NRC position on the cited vio-lations and corresponding Arkansas Power 5 Light response is provided below:
1.
NRC Position Criterion V of Appendix B to 10CFR50 requires that activities affecting quality shall be prescribed by documented procedures and shall be accom-plished in accordance with these procedures.
This requirement is further amplified by Section 5 of the licensee's NRC approved Quality Assurance Program.
Administrative Contrcl Procedure No. 1005.01, Rev. 2, T/C # 7, dated & larch 28, 1976, Section 6.1.5. A.1 states that, " Control Room logs and records are important for the information they provide for current operations and for s

July 1, 1977 1-077-2 the history of operations. All log entries, data sheet entries and chart notes must be legible, complete and understandable." Section 6.1.5.A.2 states, "The Supervisor of Operations is responsible for a review of all logs and records. He will advise Shift Supervisors of any shortcomings of operator entries and will initiate measures to eliminate or minimize the shortcomings."
Contrary to the above, the Reactor Protection System Log for April 29, 1977, was not completed as required nor were any steps apparently taken to eliminate or minimize the shortcomings.
This is an infraction.
APSL Response It is our position that the RPS Log for April 29, 1977, which was not complete, is an isolated case. We have reviewed our logs for the first and second quarters of 1977 and find no similar discrepancy to those stated in your inspection finding.
The Supervisor of Operations is aware of the requirement to review all control room logs and records. Also, plant operators have been made cognizant of your finding and have been instructed to complete all logs and records in an adequate detail.
It is our belief that, at this time, we are in compliance with Procedure l',: 5. 01.

NRC Position Technical Gpecification 6.8.1 requires that " written procedures shall be established, implemented and maintained covering... the applicable procedures recommended in Appendix A of Regulatory Guide 1.33; (and) surveillance and test activities of safety related equipment The requirement to maintain such procedures is amplified in Section 5.6.2 of the Quality Assurance Manual which requires that " Applicable instructions, procedures and drawings shall be reviewed, and revised as necessary, following modifications to the plant."
Contrary to this requirement, Procedure 1302.03 " Periodic Calibration of Incore Detectors", dated August 30, 1974, a procedure necessary to the proper perfomance of the incore detectors, was not updated to reflect a modification to the detector circuits that was perfomed in August 1975.
This is an infraction.

revised to reflect the 5,000 ohm Resistor which is used to detemine the Procedure 1302.03, " Periodic Calibration of Incore Detectors," has been CALFI factors.

Our program for incorporation of modifications into procedures is being reviewed.
It is anticipated that this review program will be completed by October 15, 1977.
