Dear Mr. Long:

Regarding the subject report and the two (2) infractions -

described therein, pursuant to Section 2.201, NRC's

" Rules of Practice" Part 2, Title 10, we wish to provide i you with the following response. They will be addressed in the order presented in the report.

Failure to Indoctrinate Personnel (Infraction A.1 -

Details I, Paragraph 2.b. (1))

Corrective steps taken and results achieved - The supervisory personnel involved with the repair of reactor coolant pump 3Al had not yet received Genera?-

Plant Employee Compliance Training which was in progress at the time the infractions occurred." On 6 May 1976 a class was held for these (and other) individuals by the Construction Mechanical Engineer on the administrative controls and documentation associated with Class I piping.

On 14 May.1976 the General Plant Employee Compliance Training Program was administered to these personnel by the Compliance Engineer with special emphasis on adherence to procedures, procedure change requirements, modification control, requirements for equipment clearance, administrative controls and documentation technique.

O As part of General Plant Employee Training, the Technical Support Engineer has administcred training to the Plant Engineers on administrative controls in the area of instrumentation and controls.

It is belie /ed that the training discussed above will avoid further occurrences of this nature. In addition, the Maintenance Engineer has been cautioned to determine that individuals performing complex maintenance operations 4

have been appropriately indoctrinated prior to beginning work. He will comply with this requirement.

Date of full comoliance - Full compliance based on the above was achieved on 14 May 1976.

Failure to Follow Procedures (Infraction A.2, Details I, Paragraph 2.b.(2))

a. Remcval of Sensinc Line 3APTl Without Written Approval Corrective steps taken and results achieved - A proper equipment clearance order (Numoer 05-114) has been taken out to isolate pressure sensing line 3A-PT1 by tagging valves RCV-60 and RCV-85 closed. A Piping Removal / Replacement Record mechanism has been incorp-orated in Procedure MP-115, which is the procedure governing the Reactor Coolant Pump Inspection and  ;

g Replacement. This mechanism provides a record, for y all removed piping, of description, clearance number, v isometric or piping drawing number, spool piece or weld number removed, replacement date, and clearance released date. This information has been updated.

The results achieved are accurate and current docu-mentation of this portion of the activity.

Corrective steps taken to preclude further noncomoliance As noted above, comprehensive training sessions in administrative and procedural controls have been administered which should avoid further noncompliances of this nature. In addition the Piping Removal /Replacemen.t mechanism will be employed in further work when removal must be performed.

Date of full compliance - Full compliance has been achieved as of 19 May 1976.

b. Procedure Chances Without Approval l

l Corrective steps taken and results achieved - The i portions of the procedure (MP-115) which were changed without appropriate review and approval have since been properly reviewed and approved by the Plant Review Committee and the Nuclear Plant Superintendent respectively.

The result of this review and approval-is assurance that the procedure, as performed, was adequate for performance of the work involved.

Corrective steps taken to avoid further noncompliance -

In addition to appropriate training and indoctrination, as noted above, a requirement has been added to the maintenance procedure to receive concurrence via Temporary Change mechanism in accordance with AI-400 Plant Operating Quality Assurance Manual Control Document. These measures should avoid further non-compliance of this nature.

Date of full compliance - Compliance has been achieved as of 24 May 1976.

c. Failure to Maintain Clean Room Status i

Corrective steps taken and results achieved - As noted in Inspection Report 40-302/76-8, immediate steps were taken to re-establish Class A :endition within the clean room. This resulting condition is satisfactory disposition of the immediate concern.

Corrective steps taken to avoid further noncomoliance -

Signs addressing clean room restrictions were manu-g-'s factured and conspicuously posted at entrances to clean rooms. These signs will be used in the future when

\s clean rooms or clean areas are established. The Construction Superintendent was directly contacted and has since instructed construction personnel of clean room restrictions. Further noncompliance with this requirement should thus be precluded.

Date of full compliance - Full compliance was achieved on 1 May 1976.

The replacement of the repaired Coolant Pump Impeller will be closely covered by Compliance Personnel to assist in preclusion of further problems. This response should provide you with the necessary information to document our actions regarding these infractions. If you have any additional questions, please advise us.

In addition, we wish to advise you that we find no areas in your reports that contain proprietary information.
