Power Station, Section 6.12.2B, we are reporting the follAs def co Nuclear occurrence as A0-74-11.

During the fill and vent procedure, ypass vent valve (SFV-26039) was cycled from 3200 gpm to 2800 gpm.the decay heat exchange throttle the discharge valve were unsuccessful Later attempts to operator revealed that(a tooth had broken from the main drivInspection of the valve object jammed between two adjacent drive gears.') The object e gear and the the main drive gear was replaced and the valve operat was removed, service.
This is the first record of a broken tooth in this type of or was returned to operator.
