An-269/75-5 Report Date: May 20, 1975 Occurrence Date: May 6, 1975 Facility: Oconee Unit 1, Seneca, South Carolina Identification of Occurrence: RIA-33 Failure During Liquid Waste Releace Conditions Prior to Occurrence: N/A Description of Occurrence:

On May 6, 1975, a routine liquid waste release was being made from a condensate monitor tank. The discharge was terminated by the control room operator when process radiation monitor, RIA-33, became inoperable.

Apparent Cause of Occurrence:

The apparent cause of this incident was water leaking from a gasket between the RIA-33 sampler and detector well into the detector junction box.

This caused RIA-33 to indicate zero counts per minute.

Analysis of Occurrence:

Technical Specification 3.9.7 requires that all waste releases be continuously monitored, with the capability to automatically terminate a release on higher than expected activities. The waste release was terminated after five minutes, with RIA-33 having been inoperable for only a portion of this time.

The condensate monitor tank, from which the release was being made, had been isolated and sampled prior to initiating the release.

The contents of the tank were determined to be within allowable limits for discharge.

Therefore, it is concluded that the health and safety of the public was not affected.

Corrective Action:

The detector junction box was repaired and the calibration of the monitor verified. A new gasket was installed between the detector wall and sampler.

Subsequent liquid waste discharges have verified the leak tightness of the system.
