APPENDIX A NOTICE OF VIOLATION i
Tennessee Valley Authority License No. DPR-77 Sequoyah Unit 1 Based on the NRC inspection March 1 through 31, 1980, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below.
These items have been categorized as described in correspondence to you dated December 31, 1974.
A.
As required b, Technical Specification 6.8.1, written procedures covering refueling operations shall be established, implemented and maintained.
Moreover, Technical Specification 6.8.3 requires that temporary changes to such procedures be documented.
Contrary to the above, on March 4, 1980, the copy of FHI-7 " Refueling i
Operation (Initial Core Loading)" which was being used to control core alterations was revision 2 instead of the current revision 4.
In addition, the requirement for continuous drag load monitoring during fuel movement specified in Section II.B of FHI-7 had been waived by plant supervision without a temporary change having been initiated.
This is an infraction.
