APPENDIX A

NOTICE OF VIOLATION Georgia Power Company License No. NPF-5 Plant Hatch Based on the NRC inspection March 11-14, and March 25-28, 1980, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in correspond-ence to you dated December 31, 1974.

A.

As required by Criterion V of Appendix B to 10 CFR 50 and as implemented by paragraph 17.2.5 of the FSAR, " Activities affecting quality shall be pre-scribed by documented instructions, procedures, drawings...and shall be accomplished in accordance with these instructions, procedures, and drawings".

Paragraph F of plant procedure HNP-904, the applicable procedure for admini-4 stration of the inservice inspection program, requires that the written document detailing the proposed work to be done be reviewed and approved prior to the outage.

Contrary to the above, during March 18-21, 1980, inspections were being performed and the Project Plan detailing the work to be done had not been approved by the licensee.

This is a deficiency.

B.

As required by Criterion V of Appendix B to 10 CFR 50 and as implemented by paragraph 17.2.5 of the FSAR, " Activities affecting quality shall be pre-scribed by documented instructions, procedures, drawings...and shall be accomplished in accordance with these instructions, procedures, and drawings".

Paragraph B. I.d. (2) of plant procedure HNP-824, the applicable procedure for control of special processes, requires that " Personnel performing nondestructive testing shall be certified in accordance with HNP-823 and SNT-TC-1A".

Contrary to the above, on March 27, 1980, welding records revealed that inspection personnel who had performed welding and NDE inspections on a modification to the Control Rod Drive System were certified by Daniel Construction Company and/or Alabama Power Company in lieu' of certification in accordance with plant procedure HNP-823 and the requirements of paragraphs 9 and 10 of SNT-TC-1A.
