Identification of Deficiency: Containment Vessel Equipment Hatch Improper Radiography Initial Report:

On February 14, 1980, Mr. C. R. McFarland of NRC Region II, Atlanta, Georgia, was notified of this deficiency by Mr. J. R. Wells of Duke Power Company, Charlotte, North Carolina, 28242.

Supplier and/or Component:

The equipment hatch was manufactured by Lakeside Bridge & Steel Company, 5300 N. 33rd Street, Milwaukee, Wisconsin, 53209, and initially radiographed by McMannus Testing Lab, a subcontractor to Lakeside Bridge & Steel Company.

Description of Deficiency:

The penetrameter was not the size as specified by the ASME Code for this thickness of material, and the penetrameter was placed on the source side of the material without the proper designation shown. The density of the radiography film was not within the specified limits of the ASME Code.

Analysis of Safety Implication:

Deficient radiography techniques could allow welds not meeting design requirements to remain undetected.

Corrective Action:

Welds were radiographed in accordance with ASME Code. Repairs to welds having unacceptable indications will be completed by May 1, 1980. Lakeside has contacted a new testing company, and Duke Power will maintain close l

surveillance to prevent a recurrence.
