Dear Mr. Irwis:

This letter refers to the apparent defbiency in the subject inspection

. report which states:

As required by Technical Specification 6.8.1.c, written procedures shall be established, implenented and maintained covering the activities con-cerning surveillance and test of safety-related equipnent. Section 3.2.9

_of Farley Nuclear Plant Mninistrative Procedure AP-5, Surveillance Program Muinistrative Cbntrol, requires that results of surveillance tests shall be reviewed by each plant staff group performing the tests to determine that all required data have been properly recorded and signed off or ini-tialled where required.

Contrary to the above, the required signatures were omitted a total of eight different places throughout STP-33.0 (Solid State Protection System Train A(B) Operability Test) which was performed on Decarber 9,1978 and STP-7.0 '(Quadrant Power Tilt Ratio Calculation) which was performed on January 22, 1979 and February 18,'1979, respectively. Although these docurents had received two levels of review, the missions had not been identified.

caragraph 6.7 of AP-5 requires provisions for initialling compliance with mandatory inspections, data sheets and significant procedure steps. he initial conditions which were not initialled provided information to the test performer but had no effect on the test validity. It was ascertained y

from the STP's themselves that the initial conditions were satisfied. 'Ihe instruction paragraphs which were not initialled required the performbr to complete an attached data sheet and make cmputations which were described on the data sheet. We data sheets in both instances were correctly and cxrpletely filled out.

to a larger nunber of staff members which will result in a nore detailed level of review.

We level of thoroughness that must be obtained in documentation and review of STP's will be discussed with licensed personnel in the licensa retraining program.

'Ihe STP's involved will be revised by April 1,1980 to delete the require-ment for initialling the steps which do not meet the "significant step" criteria of AP-5.

Such action should reduce the administrative burden and enhance the ability to perform a nore meaningful review.
