' WEEK ENDING MARCH 7,1980 Zion 1 &~2 On March 29 a Confimatory Order was issued to Commonwealth Edison (CECO) as part of NRCs effort to address potential problems caused

by the location of Zion Station in a relatively high population density area. The Order amended the Zion. licenses by incorporating and confirming CECO comitments relating to short term actions to increase.'the safety of the-plant.

Dresden Turbine Inspection A turbine inspection cf Unit 3 resulted in UT. indication of water cutting'in the keyway of the 5th wheel of the'"B" low prissure turbine. :The cutting apoears to be more severe in depth and length of cuts than that recently found at-Pilgrim. Extending the inspection to other Unit 3 turbines is still being considered.

D0R is reviewing the problem.

Report of. Incident Mitigation Assisted by Equipment Installed in Response to Short-Term Lessons Learned Requirements,

On February 4 a oower operated relief valve at. Haddam Neck opened upon receipt of a sourious signal. - The valve opening was detected by four alarm systems in the following sequence:

(1)valveposition indicator, (2) newly 11nstalled acoustic monitor in the PORV discharge oice,.(3) high pressure in the relief tank to which the PORV discharges :and (4) high temperature in the line to the relief tank.

The elapsed time of valve openina was less than 10 seconds and the drop in reactor primary system pressure was: limited to 28 psi.

Exxon Nuclear Company Fabrication of Mixed 0xide Fuel Exxon has completed fabrication of tha mixed oxide fuel assemblies for

~ Rochester Gas and Electric (RG&E) and the assemblies were shipped -

on February 25, 1980. Twelve excess mixed oxide rods are stored

'in the' Exxon;valut awaiting shipment to 00E.

1. -Proposed Regulatory Guide, " Protection of Classified Information and

. Matter," for use of NRC licensees and other NRC license related activities in the implementation of 10LCFR Parts 25 and 95 has been furnished to.the Office of Standards Development for editing and

. preparation for publication.

The Nuclear Regulatory Commission (NRC) has formally adopted two new regulations relating to the protection of'NRC classified infor-mation.at licensed facilities..These regulations, 10 CFR Part 25,

" Access Authorization for Licensee Personnel," and Part 95, " Security Facility Approval and Safeguarding of National Security Information and Restricted Data," were published in the Federal Register on March.5, 1980. These rules will become effective on May 19, 1980.

During the week of March 3, 1980, 12 individuals were~ approved for

.;RC employment prior to the completion of the required investigation and report under Section 145b of the Atomic Energy Act of 1954, as amended, under the authority delegated to the Executive Director for 3perations.by the Commission. Durin~g this same period, 30 requests from various NRC offices and divisions were received by PERSEC to

-initiate such processing.

Inceiry on the Millstone Winter Flounder Model A rep:rter fror, the States News Service called the NRC and spoke with an ESB staff ne-ter. The reporter stated that a former subcontractor for Northeast Utilities was asked several years ago by the utility to review the winter flou.cer 1: pact simulation model developed by S. Saila at the University of Rhode Isla-d. The subcontractor's conclusion was that certain values of para.ete-s used in the model were incorrect and that these incorrect values could lead to a prediction of no impact to the winter flounder fishery when, in fact, t ere might be. The reporter asked if we were familiar with the problens e th the model. The reporter was informed that the NRC assessment of imsact.o : ir.ter flounder in the vicinity of the Millstone site was performed prior to the development of the model. This assessment, using more traditional methocologi, predicted no impact and was presented in both the Fina1 Environ--

mental I-; set Statements (FEIS) for Millstone 1 and 2 and Millstone 3.

The model developed subsequent to the issuance of our impact statements supported our oredi c-ion.

To verify or refute our predictions made in the FEIS, a m:nitcrir.g program was initiated at the stations subsequent to licensing and has beer, c:llecting impingement, entrainment and far field data on the winter flour.de r.

Rece.tly, a re.'iew of impact simulation models was performed under contract wi.h : e U-iversity of Washington. The results of this effort is reported in NURE3s C:. 474, CR-0865, CR-0552 and CR-1182. One model that was reviewed in detati was t' e winter flounder model developed for the Millstone Station.

Tne cc clusion reached by our contractor in 1978 is that no presently existing ir:a:t m:cel, including the Millstone winter flounder model, can be used to make caa.titative predictions of impact.

The Mf;1st:ne modeling study was completed in 1976.

The ESB staff has since ten-fr.stec the requirement for continuance of this modeling effort at the Millstone Station based principally on the University of Washington review.

The E53 staff has placed emphasis on more empirical methods of assessing impact tc tne wir. er flounder fishery due to station operation.

It was sug;ested that the reporter contact the U.S. Environmental Protection Agen:y a c t~e Sta'e of Connecticut for additional information.

The Order is.part of the NRC effort currently underday to address, potential problars caused by the location of Zion Station in a relatively high population density area.

A press release is being issued by NRC.

Dresden Turbine Inspection On " arch 5,1980, DDR discussed with Dresden Station and Dresden NRC resident inspector the. results of a turbine inspection being performed on Unit 3.

UT indicatien of water cutting (erosion) have been found in the keyway of the 5th wheel of the "B" low pressure turbine. The cutting appears to be more severe in depth and length of cuts than was recently found at Pilgrim.

Although there were external indications. of water erosion on other wheels, there were no 0T indications in the keyways.

GE recomended that the unit be put back into service with another inspection in 9 to 12 months.

They are still considering whether to extend the inspection to the other Unit 3 turbines.

D0R is reviewing the problem and will take appropriate action prior to reactor startup which is presently scheduled for. May.

.Surry_ Unit 1 The Virginia Electric and Power Conpany is in the process of inspecting.the low pressure turbine rotor discs.

(See Daily Highlights on February 20, and 21,1980). The cause of the turbine vibration before shutdown is three broken

. turbine blades, two on row L-3 and one at the governor end of the turbine.

The disc inspections should be complete about March 10, 1980. The plant is expected to return to. power on or about March 27, 1980.

- On February 27, 1930 a caeting was' held with the Westinghouse Daners Group on F;.R Fe2d. tater Pipe Cracting to discuss their latest information on flow rodeling and possible_ corrective actions.for the pipe cracking. Mr. George

. !:hnttel is chairran of the e.:ners group. Most of the meeting was taken up by the shh.ing of a film of a water test in a plastic pipe mock-up of the feed. ster pipe at the nozzle. This mock-up simulated a short_ pipe extension design..ith a pipe bend close to the nozzle.

Test results were similar to thrse of prsvious tests. Ther.al layering of water was clearly visible in tests at.arious flows.

" Stripping," an oscillation of the interface of the hot and c:ld teater layers was also evident. This condition had been previously

. der:nstrated in a film by the study group.

Some of the remedial methods

co esed include (1) raising the temperature of the feeddater by heating with stez ar.d!(2) adding tharnal sleaves inside the feedwater pipe. The study gr0ap cxpects to complete their final report approximately mid April 1980.

FWR Feedwater Pipe Crackina - Results of Inspections and Status of On-Going Studies

n res:cnse to IE Bulletin 79-13 and an earlier NRR ietter da ed May 25, 1979, ali J. iicar.;es have inspected their feed.:ater citing in the vicinity of the stea ger:erator nozzle welds.

The licer. sees have repaired and/cr replaced the affected piping in most cases with improvec designs to minimize stress risers.

In addition, the licensees have'cc--itted to reinspect the steam generator to feedwater piping weld vicir.ities at the subsequent refueling outage.

Althcugh the piping has been repaired, the staff believes that cracking could re-occur in the future.

However, the-staff and Owners Group both have performed independent analyses and have determined that flawed feedwater piping-could withstand challenges fro. operating and faulted loads including seismic and limited water loads without loss of piping integrity.

~ The.NRC has instituted a Pipe Crack Study. Group to review this and other pipe cracking croblems in PWR's.

It is anticipated that the Pipe Crack Study. Group

'will cc.:le e.its work by June 1980 and provide reco-nendations for review and

.. Inclusien of ACRS Comments in the Draft Action Plan (NUREG-0660)

By memo dated November 26, 1979, the Chairman directed that we provide the ACRS a written summary of how the staff is responding to the ACRS recommendations included in their August 14, 1979 letter.

In developing draft 2 of the Action Plan the staff determined the particular sections of that document which responded to the various ACRS recommendations. This was done not only for the August 14 letter, but for all ' ACRS letters issued between March 28, 1979 and January 1,1980.

Report of Incident Mitigation Assisted by Equipment Installed in Response to Short-Tern Lessons Learned Recuirements Cn Monday, February 4,1980, a power operated relief valve (PORV) at Haddam

- Neck ' opened upon receipt of a spurious signal. At the time of the incident, the rea: tor was operating at 100 percent power and a primary coolant pressure of 2323 osig.

The valve opening was detected by four alarm systems (each of which gave audible and visible alarms) in the following sequence: (1) valve position indicator (stem mounted ~ limit switch),-(2) the newly installed ac'oustic monitor in the PORV discharge pipe, (3) high pressure in the relief tank to which the P3R7 discharges, and (4) high temperature in the line to the relief tank and

(*) highlecperature in the line to the relief tank. The acoustic monitoring sys en was ' installed in December 1979 in response to Lessons Learned Requirement 2.1.3.h.

After detection the operators switched the PORV to the closed position and the valve closed.

The elapsed time of valve opening was less than ten seconds and the drop in reactor primary system pressure was limited to 28 psi.

Following the incident the PORV was place " hack in automatic and the down stream block valve was closed.

CYAPCo has not yet determined the cause of the spurious signal.

They will inform us of the results of their investigation of the cause.

Dublication of Proposed Rule to Delete Source 'iaterial Medicinals from the General License for Small Quantities of Source Material On March 6,1980, a proposed amendment to Part 40 was published in the r deral Register under the signature of the Acting Executive Director e

for Operations. This amendment would delete the provision which authorizes the use of source material medicinals by physicians, pharmacists and other aersons receiving the source material in the form of medicinals or drugs.

This proposed action would prohibit any internal or external administration of scurce material, or the radiation therefrom, to human beings, except where authorized by NRC specific license. The 60-day comment period ends on May 6,1980.

Karl Goller, Ralph Jones, and 5teve McGuire met with representatives of NMSS, IE, AEOD, ELD, and SP at the first meeting of the Radiography Safety. Steering Committee. Twelve action items were discussed and agreements reached on lead organizations for the items to expedite the program. The program was initiated as a

Other meetings with -licensees are planned to provide further information on implementation of the Upgrade Rule requirements.

Numec Acolic Safecuards Study The,Heuse Committee on Interior and Insular Affairs has commented favorably on the staff's analysis comparing safeguards as they now exist to what they were in the mid-1960's at the Numec facility.

Meeti n: with Amarican Telechone and Telegraph Members of American Telephone and Telegraph (AT&T), and their subsidiaries, met'with staff of Material Safety and Licensing to discuss the licensing of

The telephone cc=: aries anticipate a need for 10 million devices in their cffices by 1985.

NRC staff anticipates that AT&T will be requesting an exemption from licensing for these devices.

Exxon Nuclear Company Fabrication of Mixed Oxide Fuel

. A representative of Exxon Nuclear Company has informed us that they have completed fabricating the mixed oxide fuel as:emblies for Rochester Gas and Electric Co. (RG&E) and the assemblies were shipped from Exxon to RG&E on February 25. 1980. The 12 excess mixed oxide rods obtained from Westinghouse

'PFDL are nmv stored in the Exxon vault. Exxon expects to remove the mixed oxide fuel. from these tubes and ship the fuel to DOE, as arranged by RG&E.

Exxon also has set an objective to dispose (hopefully to DOE) of all plutonium in their possession at Richland by November 1980.

Onsite Burial of Waste On March 4, Exxon Nuclear, Richland, Washington, called NMSS to discuss onsite burial of waste containing low-enriched uranium in accordance with the pro-visions of 10 CFR 20.304. Exxon was_ informed that consideration was being given to eliminating the burial provisions from Part 20 and that onsite burial -

was being discouraged. Exxon stated that with NEC0 Richland refusing to accept any special nuclear material, Exxon has reached.the point that they are choking on their internally generated waste.

General Electric Morris Operation - License Renewal A prehearing conference was held on February 29, 1980, for the proposed renewal of General Electric's Materials License No. SNM-1265. This license authorizes GE to receive, store and transfer spent fuel at their Morris Operation in Morris, Illinois.

Two intervening parties, the State of Illinois and a group of concerned ~ individuals, Rorem, et al., have been given until March 20, 1980, to restate their contentions. A hearing schedule has not been determined.

Duke Power - Oconee/McGuire Transshipment The proceedings for the amendment of Duke Power Company's Materials License No.

SNM-1773 will reconvene on April 28, 1980. This amendment would authorize the receipt.of 300 spent fuel assemblies from Oconee Nuclear Station.at McGuie Nuclear Station.

The hearings had been delayed as a result of questions

. regarding the disclosure of alternate routes.

By Order, On February 29, 1980, the Commission directed the staff to release the routing information.

Remaining topics to be litigated include the environmental impacts associated with the use of approved ~ alternate routes, and cotoliance with the new regulation 10 CFR 73.37 requiring protection of irradiated fuel in transit.

Toledo Edison Company (Davis-Besse Unit 1) - On March 5,1980, an Order Modifying License was issued to the subject licensee. The order, which

.is effective immediately, requires the licensee to take certain actions regarding the assignment and training of-personnel.

Gorsira X-Ray, Inc., Farmington Hills, Michigan - On February 28, 1980, an Order to Cease and Desist was issued to.the subject company. The

company was required to cease and desist immediately from further use of sealed radiographic sources; store all byproduct material in its possession; and promptly transfer the byproduct material in its possession to a person authorized to receive it. This action was taken because the company was possessing and using byproduct materials.for the conduct of radiographic operations without authorization from the NRC.

American X-Ray & Inspection, Inc., Farmington Hills, Michigan - On February 28, 1980, an Order to Show.Cause and Order Suspending License was issued to the subject licensee. This action was taken because of alleged

-items of noncompliance relating to excessive radiation levels in unre-stricted areas during radiographic operations and because the licensee transferred. control of NRC Materials License No. 21-15455 and licensed raterial to Gorsira X-Ray, Incorporated, a non-licensee.

The following Notification of Significant Enforcement Action was dispatched:

EN-80-06 University of Chicago, Chicago, Illinois - On March 3,1980, a Notice of Violation and a Notice of Proposed Imposition of Civil Penalties in the amount of $2.400 was issued to the subject licensee.

This action is based on eight alleged items of noncompliance including the whole body exposure of an individual to 14.3 rems and an extremity exposure to that individual of 45 rems.

Surry Nuclear Power Station, Units 1 and 2 On March 4,1980, the Commission issued a Memorandum and Order with respect to three lQ CFR 5 2.206 decisions of the Director of the Office of Nuclear Reactor Regulation involving the proposed steam generator repair at the Surry Nuclear Power Station. The Commission directed that the NRC staff expeditiously prepare and issue an environmental impact statement on the proposed repair at Unit 1.

Three Mile Island Nuclear Station, Unit No.1 On March 6,1980,- the Commission issued an Order (CLI-80-5) in which it provided specific guidance to the presiding Licensing Board as to the issues to be considered in the hearing with respect to the management of Metropolitian Edisor Company.

.e ITEMS OF INTEREST OFFICE OF INTERNATIONAL PROGRAM 5 WEEK ENDING MARCH 7. 1980 INTERNATIONAL COOPERATION Soviet Nuclear Accident The January-February 1980 issue of Nuclear Safety (Vol.21, No.1) carries a 4-page article entitled " Technical note: The 1957-1958 Soviet Nuclear Accident in the Urals," which challenges the conclusion (reached by the authors of a Science, 206, article) that the " accident was the result of unusual localized f allout from a Soviet atmospheric nuclear weapons test." The three who wrote this N.S. article were among the five co-authors of "Another Perspective of the 1958 Soviet Nuclear Accident," (Nuclear Safety, 20(2))which examined the very' limited information available and concluded that there was a major accidental airborne release of fission wastes, although.its exact nature and consequences coul.d not be defined.

EXPORT / IMPORT AND INTERNATIONAL SAFEGUARDS Dr. Shibata, Dr. Kanda, and Mr. Miyabayashi accompanied by other representatives of. Nissho-Iwai Corporation visited with IP to discuss the program for Reduced Enrichment for.Research and Test Reactors conducted by.the U.S., Japan, and other countries around the world.

The discussion centered around the status of the reduced enrichment program at Kuca critical assembly and the Kuhfr reactor in Japan and the presently. pending export application for 42 kg of HEU, for which.the-Japanese have urgently requested an export license by March 31.

Executive Branch views were received in the last few days and the case is being expedited to the Commission.
