Item 1
'During-the In-Service Test (IST) program review with the NRC on January 30-31, 1980, the Event V isolation valve configurations at Salem were identified.
_There are five (5) basic configura-tions with_ twenty (20) paths from the High Press'ure Reactor Coolant System (RCS) to the Low Pressure Injection System (LPIS) piping.
These are shown in the attached Figures 1-5.
Although the configuration _in Figure 5 does not conform to the descrip-tion in the' referenced letter, it was discussed at the IST pro-l gram review and specifically addressed by the'NRC as valves per-forming a pressure isolation function.
-Item 2 Currently, there is'no specific surveillance program to insure these valves are not leaking.
All the valves are periodically exercise ~d in accordance-with the IST program to' insure oper-ability.- Leakage through the pressure isolation paths would be
' indicated by an increase'in RCS make-up.
- A review of completed: Surveillance Procedures and Equipment History Files reveals no record of loss of integrity for the b
valves under discussion..
