DUKE POWER COMPANY OCONEE UNIT 1 g
Report No.: A0-269/75-14 Report Date:
December 11, 1975 Occurrence Date:
November 27, 1975 Facility:
Oconee Units 1, 2, 3, Seneca, South Carolina Identification of Occurrence: Release of tritium in excess of allowable limits Conditions Prior to Occurrence:
Not Applicable Description of Occurrence:
On Thursday, November 27, 1975, the contents of Oconee Condensate Test Tank "B" were released at a flow rate of 46 GPM for 36 minutes.
This rate exceeded the maximum recommended release rate of 30 GPM as determined by Station Health Physics personnel and resulted in a tritium #*1****
f 3
3 3.62 X 10 uC1/ml.
This value exceeds the limit of 3 X 10 uCi/ml determined pursuant to Technical Specification 3.9.3.
Apparent Cause of Occurrence:
The occurrence was apparently caused by personnel error. Upon receipt of the recommendation for the maximum release rate, the operator involved misread the value as 50 GPM instead of 30 GPM as stated on the form and subsequently set up the release from the condensate test tank with a maximum flow rate of 50 GPM instead of 30 GPM.
Analysis of Occurrence:
This incidgnt resulted in a tritium release to the environment of 0.62 X 10 LCi/ml above the specified limit for a period of 36 minutes.
(Other activity released was within limits.)
This value is sufficiently low as to not constitute a hazard, and, therefore, it is concluded that the health and safety of the public was not affected by this incident.
3 Corrective Action:
This incident has been discussed with the personnel involved. The importance of carefully reading and interpreting all information relating to operations of this nature has been carefully stressed.
