Sargent & Lundy Engineers Docket No. 99900507/80-01 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on March 24-28, 1980, it i
appeared that certain of your activities were not conducted in full compliance with NRC requirements as indicated below:
Criterion V of Appendix B to 10 CFR 50 states:
" Activities affecting quality shall be prescribed by documented instruc-tions, procedures or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, proce-dures or drawings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for deter-mining that important activities have been satisfactorily accomplished."
Deviations from there requirements are as follows:
A.
Sargent & Lundy General Quality Assurance Procedure GQ-3.07, Revision 4, (8/21/78) state; in part:
"If the Project Leader.
. determines that no interface -xi.its the drawing shall be forwarded directly to the Reviewer. The decision that no interface condition exists shall be docuoented... The Project Leader shall maintain a log containing information as to whom and when the drawing was sent for comments and when the comments were received. All comr.ents and comment' resolution documents shall be maintained in file for at least one year or until the next drawing revision has been issued."
Byron /Braidwood Project Instruction PI-BB-5, Revision 5, (9/5/79) Mechan-ical Department Drawing Review and Comment Requir.ments states in part,
" Prior to initial alpha release of a design drawing issued for construction or fabrication, comments shall be solicited from departments and divisions as indicated in Table I."
Contrary to the above, no comments were solicited for the initial alpha release of the following drawings and no decision that an interface did not exist was documented.

Also, on drawing M2098-01 thru-07 HVAC/C&I Diagram Diesel Generator Room Ventilatioc System - VD Unit 2, Revision A, 28 March 79, the drawing was sent out fo: comments, comments were received, but the date they were received was not entered on the record of transmittal which was used for the log.
B.
Sargent & Lundy General Quality Assurance Procedure GQ 3.10, Revision 4, (9/20/78) states in part:
"If the design review report notes any system or structure design discrepancies, the Project Manager / Senior Project Engineer (s) Review Coordinator shall initiate the action to correct these discrepancies... The department standard shall describe the method for verifying and documenting that_the correction of these discrepancies has been completed... The system and structure design review status report shall show when the review of a system or structure is completed."
Contrary to the above, the department standard for the Mechanical Depart-ment MAS-4 does not describe a method for verifying and documenting when correction of outstanding discrepancies has been completed.
For example, the system and structure design review for the Main Feed-water System had outstanding discrepancies which had not been closed out; yet the design review report and the System and Structure Design Review Status Report showed the design review complete.
