Narrative

Report for LER 50-366/1980-059 With the unit in cold shutdown, an inspection of MSIV limit switches was performed in response to NRC concerns.

One non-qualified outboard MSIV, 2B21-F028A, was found to have a Namco model EA700 installed in the 90%

open location.

According to the limit switch retrofit manual, drawi#g no.

S-30830, all MSIV limit switches in the 90% open location were to be replaced with a qualified Namco model EA740, and all were to remain.

A limit switches in the full closed locac3-a qualified limit switch was found in the full closed location on MSIV 2B21-F028A.

Therefore, it is' believed that during the retrofit imp l eracn t a t i on,

the full closed limit switch on 2B21-F028A was replaced instead of the 90% open limit switch.

These two limit switches were interchanged prior to startup.

The limit switch retrofit manual was issued for Unit 2

in response to IEB 79-01.

Also in response to IEB 79-01, an inspection of the Unit 1 MSIV ' limit switches was performed.

All limit switches were found to be non-q ua li f i ed, and this was reported by LER 50-321/1979-037.

Namco model EA740 limit switches were installed on Unit I u-ver DCR 79-171 during the spring outage of 1979.

The MSIV limit switches have, no direct backup.

The non-qualified limit switch on 2B21-F028A was found to be functioning p r o p e'r l y,

so no unsafe condition existed due to this event.

This event caused no threat to public health or safety.
