Dear Mr. Keppler:

On December 28, 1979, Messrs. R. E. Turner and T. D. Geib of Public l Service Company of Indiana, Inc. (PSI) telephoned your office to notify you of a potentially reportable incident as required by 10CFR 50.55(e).

i The following report resolves that item and constitutes a final report under the Regulation. In late December 1979, PSI Quality Assurance personnel discovered several joints in safety related structural steel located in the Auxiliary Building which had not been bolted properly.

Standards governing structural steel erection require plate washers to be used when bolting joints with long slotted connection holes.

These washers must be large enough to cover the entire slotted hole.

The structural steel joints in question either had washers which were not large enough to cover the entire slotted hole or did nM have any washer over the slotted hole at all. Some of these structural steel joints had also been inspected and approved by contractor quality assurance personnel.

i Absence of plate washers on long slotted connection holes can modify the way a joint reacts to structural loading. While the struc6 tre is l conservatively designed, failure of the joint may result if it is subjected to the extreme loadings which may occur during certain design basis events.

As discussed in the interim report of January 24, 1980 PSI has l developed and is presently implementing a procedure under the Construction Verification program to verify the proper installation of washers in all sr.fety related structural steel joints. This procedure, Special Process Procedure 3 (SPP-3), will serve to identify those 56

af PUBLIC SERVICE Letter: Mr. James G. Keppler April 25, 1980 INDIANA joints which do not have washers properly installed and is enclosed with this letter. All structural steel joints which do not have the proper washers will be reinstalled with the correct washer configuration.

This reinstallation will be done in accordance with approved ccntractor installation procedures.

To prevent the reoccurance of this situation, contractor inspection procedures will be revised to incorporate the characteristics of verification procedure, SPP-3 by July 1,1980.
