Arkansas Nuclear One, Unit 1 Docket No. 50-313 APPENDIX A
'il0TICE OF VIOLATION Based on the results of the NRC inspection conducted February 10-11 and February 15-18, 1977, it appears that certain of your activities were not conducted in full compliance with your Reactor License or NRC requirements as indicated below:
1.
Criterion XVII of Appendix B to 10 CFR 50 requi-es in part that,
" Sufficient records shall be maintained to furnish evidence of activities affecting quality...
Inspection and test records shall, as a minimum, identify the inspector or data recorder, the type of observation, the results, the acceptability, and the action taken in connection with any deficiencies noted."
This requirement is further amplified by Section 11.4.2 of the licensee's NRC approved Quality Assurance Program Manual for Operations which also requires the inclusion of the date of the test.
Contrary to the above:
a.
Attachment F (Main Fuel Bridge Checkout) of Procedure 1502.03 (Preparation for Refueling) which was performed on or about February 14, 1977 was not dated.
b.
The data sheet for Procedure 1304.28 (Calibration Check of Radiation Monitors) which was performed on or about February 2, 1977 was not dated or signed by the data taker.
This item is a deficiency.
