Licensee Action on Previous Inspection Findings

(Closed) Open Item (440/77-05; 441/77-05) Justification for samples or

analyses not performed. The previous RIII report discussed the desir-

ability of including justification in the environmental monitoring

reports for those samples or analyses which were not performed or not

reported. During this inspection the RIII inspector reviewed the monthly

sampling and analytical results of water quality transmitted in reports

from NUS Corporation to CEI, from September 27, 1977 (covering surveys of

June through August 1977) through January 4, 1980 (covering surveys during

September through November 1979). The inspector noted that, in all cases

where sample results were missing, explanatory notes had been added to

the report sheet. This item is considered to be closed.

(Closed) Open Item (440/78-10; 441/78-09) Resolution of erosion problems

at the sediment control overflows. Through discussions with the licensee

and review of the environmental monitoring reports for Perry, the RIII

inspector determined that the erosion problems at the three sediment

impoundment overflows have been solved by the construction of concrete

dams to eliminate the rip-rap. The work was completed in April 1979.

This item is considered to be closed.

(Closed) Open Item (440/78-10; 441/78-09) Monitoring of groundwater

levels. The previous RIII report noted a CEI commitment to monitor

groundwater levels biweekly at the remaining piezometer locations.

Review of Licensee Action on Previous Inspection Findings

The RIII inspector reviewed the licensee actions relative to the

resolution of specific open items which had been identified in

previous RIII inspection reports, and which were still in an open

status prior to this inspection.

The items reviewed, and the

licensee actions relative to the resolutions of each item, are

discussed in the foregoing section of this report.

2.

Qualification of Inspection, Examination and Testing Personnel

The RIII inspector reviewed the qualifications and certifications

of selected CEI, Kaiser Engineers (KEI) and Gilbert Associates, (GAI)

personnel engaged in inspection, examination, and testing activites,

with emphasis on the comparison of the personnel qualifications to

the requirements of ANSI N45.2.6 and the applicable Perry Plant

procedures.

CEI

The inspector reviewed CEI Program Quality Assurance Procedure

1-1001, Revision 0, dated January 26, 1979, " Qualification and

Certification of CEI Inspection and Test Personnel," and the

qualification records of nine CEI personnel, including four in-

dividuals certified to Level III and five persons certified to

Level II.

KEI

The inspector reviewed KEI Quality Assurance Procedure No. 20,

Revision 2, dated November 15, 1979, " Indoctrination, Training,

and Certification", and the qualification records of 25 KEI per-

sonnel, including 23 persons certified to Level II and two in-

dividuals certified to Level III.

GAI

The inspector reviewed the following GAI procedures related to

qualification of inspection, examination and testing personnel:

The inspector also reviewed the qualification records for 13 GAI

Based on the review of the above procedures and qualification records,

and discussions with CEI, KEI, and GAI quality assurance personnel, the

inspector concluded that the procedures and personnel certifications

reviewed meet the intent of ANSI N45.2.6.

Observation of Activities

The inspector observed that welding of Unit No. I recirculation N2

safe-end was being performed in accordance with the approved proce-

dures, that welding activities were being performed and protected

from the elements, that joints being joined were clean of all foreign

matter, and that all equipment was being checked and properly

maintained.

Unit No. 2N2 recirculatior. safe-ends were being removed and weld

preparation machined in accordance with approved procedures; the

finished surfaces covered for protection and to prevent the entrance

of foreign objects into the vessel.

No items of noncompliance or deviations were noted.

Though all documentation for the safe-end replacements have not

arrived from GE, the material certifications and NDE examination

Exit Interview

The inspectors met with site staff representatives (denoted under Persons

Contacted) at the conclusion of the inspection on January 25, 1980. The

inspectors summarized the purpose and results of the inspection. The

licensee achnowledged the findings reported herein.
