O APPENDIX A NOTICE OF VIOLATION Public Service Electric & Gas Company Docket Nos. 50-354 50-355 This refers to the inspection conducted by representatives of the Region I (Philadelphia) office at the Hope Creek Generating Stations, Units 1 and 2. Lower Alloways Creek Tcwnship, Salem County, New Jersey, of activities authorized by NRC License Nos. CPPR-120 and CPPR-121.
During this inspection, conducted November 26-30, 1979, one apparent item of noncompliance was identified:
10 CFR 50, Appendix B, Criteria V, states in part, that:
" Activities affecting quality shall be prescribed by documented instructions, procedures or drawings...and shall be accomplished in accordance with these instructions, procedures or drawings."
Special work plan procedure SWP/P-C-1, entitled, " Concrete Operations",
provides instructions for documenting concrete repairs and states, in part, that:
"When repairs are expected to be delayed beyond an 8 to 12 week period, the original pour card may be closed out and filed by referencing a new pour card established to document the repair work...The Lead Civil Engineer shall develop and implement a filing system which maintains the current status of all concrete repairs."
Contrary to the above, as of November 27, 1979, the Lead Civil Engineer had not implemented an effective filing system to maintain the current status of concrete repairs. Additionally, field engineering had not issued a supplementary pour card on delayed repairs where concrete voids extended beyond the depth of the first layer of reinforcing and the original pour card had been closed.
