Appendix A NOTICE OF VIOLATION Toledo Edison Company Docket No. 50-346 Based on the inspection conducted on January 2 thru 4, 7 thru 11, 14 thru 18, 21 thru 25, 29 thru 31 and February 1, 1980,-it appears that certain of your activities were in noncompliance with NRC requirements as noted below. This item is a noncompliance of the deficiency level.
Technical Specification 3.7.6.1 requires two independent Control Room Emergency Ventilation Systems to be operable in Modes 1, 2, 3 and 4.
The surveillance portion'of that specification (TS 4.7.6.1.e.3) requires a makeup flow of 300 cfm + 10% when supplying the Control Room with outside air.
Lineup for Control Room Emergency Ventilation System #1 requires that switch HA5261A on cabinet CDE11C be in the remote position.
Contrary to the above on January 24, 1980, while in Mode 1, the inspector found switch HA5261A on cabinet CDE11C in the local position.
This switch alipment would have prevented the Control Room Emergency Ventilation System #1 to be operated from the Control Room to provide a supply of outside air.
