Areas Inspected:
Inspectior. of safety-related hangers and restraints
including review of anchor t olt installation specification, observation of
snubber installations, and review of S&L document control provisions. The
inspection involved a total of 18 inspector-hours onsite by one NRC in-
spector.
Results: Of the three areas inspected, no apparent items of noncompliance
were identified in two areas; one apparent item of noncompliance was iden-

Licensee Action on Previous Inspection Findings
(Closed) Noncompliance (373/78-25-01; 374/78-16-01) - Inadequate specifi-

cation for procuring mechanical snubbers. This item was reviewed previous-
,
ly, see Report Nos. 373/79-04; 374/79-01. The inspector reviewed the ap-
proved S&L ECN's M-145-LS and M-137-LS, and considered the corrective
action adequate. The inspector stated that he had reviewed the PSC
snubber functional test and environemntal test reports previously at GE
and at PSC offices, and had no adverse comments. The timely updating of
design specification and ECN reference requirements in the applicable
specification are discussed in Paragraph 3 of this report.
(0 pen) Unresolved Item (373/79-19-01; 374/79-13-01): No CECO audit on-
S&L pipe whip restraint design.
CECO performed an audit on October 1-3,
1979, at S&L.
The audit report was issued on October 15, 1979 . The
inspector reviewed the content of the report and considered the findings
to be significant. The licensee stated that S&L had responded.to the
CECO findings and a CECO followup audit at S&L is scheduled for the end
of February, 1980. The final resolution will be reviewed by the inspector.

(Closed) Noncompliance (373/78-25-03) Inadequate hanger QC inspections.
'
A followup inspection was performed and documented in RIII Report No.
50-373/79-19. This item is now considered resolved based on the inspec-
tion results documented in Paragraph 2 of this report.
(Closed) Noncompliance (373/79-04-01; 374/79-01-01) Inadequate concrete
anchor bolt inspection program. This item is closed, see Paragraph 1 of
this report for details.
(Open) Noncompliance (373/79-04-02; 374/79-01-02) Insufficient concrete
anchor bolt rework. During this inspection eight 3/4" bolts on M09-RH14-
1046S and six 5/8" bolts on M09-RH14-1052S were tested and all met the
test torque requirement per ECN 1043, December 10, 1979.
During the
hanger inspection many bolts were observed out of plumb, cut, and with
excessive projection. The licensee stated that 100% re-inspection will
be performed on all safety related concrete anchor bolts.
Inspection
area will include:
(1) edge distance, (2) bolt spacing, (3) embedment
length (or projection distance), (4) out of plumb, (5) torque testing,
and (6) inspection of cut bolts. The inspector will review the inspection
and rework procedure during a subsequent inspection.
(Closed) Noncompliance (373/78-25-05; 374/78-16-04) Inadequate control of
hanger component outdoor storage. This item was inspected previously,
see RIII Report No. 50-373/79-04 and 50-374/79-01.
Extensive inspection
on the condition of each component in the out door storage areas was
conducted by the site CONAM organization.
The checklist used and the
documentation of findings are considered to be acceptable, however, there
was no formal procedure established for instructing who, how, and how
frequent the surveillance, inspection, and rework should be performed.
During the inspection, a Ceco Site Quality Procedure LSQP No. 13-2,
Revision 1 dated January 31, 1980, was written, reviewed, approved, and
issued for use.
The inspector reviewed this procedure and had no adverse
consents.
Functional or Program Areas Inspected
1.
Review of Anchor Bolt Specification
The inspector reviewed the S&L ECN No. 1043, documenting Revision 5
of Form LS-CEA, " Standard Specification for Concrete Expansion
Anchor Work", dated December 10, 1979.
In discussion with the site
engineering representative, the inspector was told that (1) no
sleeve type concrete expansion bolts were used in safety related
concrete or block walls, and (2) no lubrication was applied during
bolt installations.
After the specification review, the inspector
commented:

The engineering basis or test data used to develop Table 38-6,
" Installation Torque, ft.-lb. for Anchor Installed in Minimum
3500 PSI concrete", was not available for review.

crete", was uncertain.
This is considered an unresolved item.

Verification of llanger QC Inspection
The inspector observed the following safety related snubber install-

The inspector also observed some of the shims installed at pipe
clamps where large gaps were identified between the shear lugs and
the clamp. The work was done in accordance with the ECN No. 153,
dated August 17, 1979 requirements.
No items of noncompliance or deviations were identified.

S&L Design Specification and Drawing Change tontrol
During his review of hanger and restraint activities the inspector
observed the following:

Unresolved Items
Unresolved items are matters about which more information is required in
order to ascertain whether they are acceptable items, items of_noncompli-
ance, or deviations. An unresolved item disclosed during this inspection
is discussed in Paragraph 1.
Exit Interview
The inspector summarized the scope and findings of the inspection during
a telephone conversation with licensee representatives on January 31, 1980.
The licensee acknowledged the findings reported herein.
