Systems and Services Division Docket No. 99900379f79-01 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on October 15-19, 1979, it appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below:

Criterion V of Appendix B to 10 CFR 50 states:

" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.

Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished." Deviations from these requirements are as follows:

A.

Paragraph 6.00 of Standard Practice Instructions No. QA-C-21, dated November 29, 1967, states in part, "The Production Test Specification will be based upon the written report (5.00). The procedure and equipment will be completely and clearly presented on Form #1169, the final copy of which must be approved by the Supervisor of Product Quality. The original will be filed in the Quality Control Department.

Contrary to the above, the Production Test Specifications for Piston Damper Operator, Code No. D-3000, and VC3752 Valves, Part Nos. 17-287-t Suffix, and 16-25XXX-Suffix had not been approved by the Supervisor of Product Quality. The Production Test Specifications were dated July 20, 1972, and May 8, 1976 (latter two), respectively; and were located in the Quality Control files.

B.

Paragraph 2.01.6 of Standard Practice Instructions No. QA-T-53, dated August 30, 1979, states, "Each data sheet shall list the instrument number, brief description of the instrument, and the calibration check dates."

Contrary to the above, the instrument number, brief description of the instrument, and the calibration check dates had not been listed on three (3) data sheets for a Controller, Part No. 27-2255-18, which had been tested on October 15, 1979.

The required information was added to the data sheets prior to the NRC inspector's departure from the facility.

In addition, the NRC inspector was provided a copy of the revised data sheet which incorporates provisions for instrument information. This is deemed adequate corrective action and preventive measures; therefore, written response to this deviation is not required.

Paragraph 3.01 of Standard Practice Instructions No. QA-C-15, dated August 18, 1967, states in part, " Write the procedure (Inspection Process Control Sheets (IPCS)) as completely and clearly as possible, using illustrations wherever necessary. After typing, submit to your supervisor for approval.

Place the original in the IPCS Master Book...."

Contrary to the above, Inspection Process Control Sheets (IPCS) located in the Master Book had not been approved by the supervisor for the following Part Nos. : 34-119-9, IPCS dated February 12, 1976; 34-121-6, IPCS dated February 12, 1976; and 14-255-4, IPCS dated August 8, 1978.

D.

Paragraph 4.5 of Standard Practice Instructions No. 57-21, dated November 1, 1972 states, "The department assigned corrective action will take steps to insure that the reason for the deviation is eliminated, by issuing or requesting an Engineering Change-Order, or by correcting tooling, manu-facturing, or vendor methods. The department will briefly summarize corrective action taken on its copy of Form 1137 (bottom half) or AVO and forward same to Quality Control."

Contrary to the above, the department assigned corrective action had not briefly summarized the corrective action taken on the bottom half of its copy of Form 1137 or AVO and forwarded same to Quality Control as evidenced by the following Forms 1137 located in the Quality Control files: Nos.

10990 dated May 29, 1979; 10892 dated May 4, 1979; 10193 dated December 20, 1978; and 10922 dated April 22, 1979.

E.

Paragraph 3.13.2 of Engineering Drafting Department Standard No. EC-1020, dated December 1976, states,

" Checked By: The signature (written) of the person checking the completed E.C.O.

This box will also be initialed by the manager."

Contracy to the above, the " checked by" box, of the following completed Engineering Change Orders (ECO) had not been initialed by the Manager:

Nos.77-102 dated February 16, 1977;78-250 dated June 2, 1978;78-254 dated June 26, 1978;79-183 dated June 11, 1979; and 79-252 dated August 1, 1979.

In addition, the name of the checker was printed rather than written on Nos.77-102, 78-250, and 78-254.
