~The Anaconda Company Continental Wire and Cable Corporation Docket No. 99900372/79-01 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on August 7-10, 1979, it
- appears that certain of your activities were not conducted in accordance with NRC requirements.
l Criterion V of Appendix B to 10 CFR 50 states: " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a
~ type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
Instructions,_ procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished." Deviations from these requirements are as follows:
A.
Paragraph 1.5.1 of the Topical Report AWC-75-A, Revision 1, dated November 18, 1976 states, "Each plant shall have a Quality Assurance Manager who shall report to the Plant Manager for the administrative operation of his department. He shall have a clear access to the Plant Manager, sufficient authority to ensure that quality requirements are consistently. maintained, and freedom to make quality decisions without pressure or bias."
Paragraph 1.11.1 of the Topical Report AWC-75-A, Revision 1, dated November 18, 1976 states, " Manufacturing facilities may consolidate or divide organizational responsibilities so long as function, systems and authorities as described within this manual are maintained. Levels of reporting shall be equivalent to those described within this manual."
Additionally,~ Exhibit No. 1 of the Topical Report AWC-75-A, Revision 1, dated November 18, 1976, depicts the Plant Manager as having total responsibility of the manufacturing facility.
Contrary to the above, the Quality Assurance Manager does not report to a level equivalent to the Plant Manager inasmuch as the individual to whom the QA Manager reports does not have total' responsibility for the manufacturing facility. See Details Section, paragraph C.3.a.
B.
Paragraphs 2.4.1 and 2.4.5 of the Topical Report AWC-75-A, Revision 1, dated November 18, 1976, states in part, respectively:
" Inspection procedures are issued by the Power Cable Manager of Quality Assurance to each manufacturing plant.
" Inspection procedures will be reviewed and approved by affected plants prior to issue."

2 Contrary to the'above, Inspection Procedure No. 524, dated April 6, 1978, had"not been reviewed and approved by the affected plant (York),
prior to issue.
C.
Paragraphs 2.5.1 and 2.5.5 of the Topical Report AWC-75-A, Revision 1, dated November 18, 1976, states in part, respectively:
'" Test Methods are issued by the Power Cable Manager of Quality Assurance to each mamdactur1ng plant.
" Test methods will'be reviewed and approved by affected plants prior to issue."
Contrary to the above, the following Test Methods had not been reviewed-and approved by the affected plant (York), prior to issue:

Paragraph 3.2.3 of the Topical Report AWC-75-A, Revision 1, dated November 18, 1976 states in part, " Prior to issuing the copies... the MSO, EN or SI is checked for accuracy of design and specification require-ments and is initialed as approved by the Chief or Senior Specificatica
' Engineer. The Quality Assurance Manager will also review the MSO, EN or SI for accuracy and completeness of quality assurance requirements. He will then initial the MSO, EN or SI as approved in the area provided for approval."
Contrary to the above, the following Engineering Instructions (EN) for nuclear orders had not been initialed as approved by the Chief or Senior Specification Engineer or Quality Assurance Manager:

Paragraph 5.1 and its subparagraph a. of the Topical Report AWC-75-A, Revision 1, dated November 18, 1976 states, respectively:
" Controls are established to monitor production at various stages of manufacture. These controls are continually in effect and are outlined in either the Manufacturing Standards or Inspection Standards.
" Written procedures for in process control of fabrication are employed."
-Manufacturing Instructions No. 8, Section 0, dated August 2, 1976 is an Index of instructions applicable to extrusion equipment and identifies seventeen (17) instructions.
Contrary to'the above, the. seventeen (17)-procedures (instructions) identified in the Index, for in process control of the extrusion operation had not_been prepared.

.' Paragraph 3.5.1 of the Topical Report AWC-75-A, Revision 1, dated November 18, 1976 states, "All changes' to MSO, EN or SI are reviewed by, the Chief or' Senior Specification Engineer and initialed as approved prior to being distributed. All changes which effect materials, design
. specifications and/or Quality Assurance requirements are also reviewed and initialed as approved by the Quality Assurance Manager prior to being distributed."
