Dear Mr. Engelken:
In accordance with Section 2.201, Title 10, Code of Federal Regulations, the Sacramento Municipal Utility District hereby responds to the Notice of Violation received by the District on June 6, 1978.
The Notice was issued as a result of the findings contained in IE

Inspection Report 50-312/78-05.
Appendix A of your letter notices the following deficiency:
Contrary to the requirements of the Rancho Seco Technical Specifications, Paragraph 6.10.1.d, the surveillance testing of the redundant high pressure injection recirculation valve, SFV-23646, was not documented when SFV-23645 was removed from service for inspection on April 6, 1978.
SMUD REPLY During this time period, three breaker inspections were being perforced on equipment to comply with IE Bulletin 78-05.
Examinations of records relating to removing cquipment from service indicated that appropriate documentation for valve lineups and surveillance of redundant components, with the exception of the valve in question, were performed.
Log entries for April 5 indicated the "B" NSCW pump was tested prior to the "A" NSCW pump being removed from service for the breaker inspection.
Upon completion of the breaker inspection log entries noted that the "A" NSCU was tested to assure operability prior to removal of the "B" NSCW pump from service on April 6.
Upon completion of the breaker inspection on April 7, ' the "B" NSCW was again tested to assure operability.

Log entries on April 6 indicated that SFV-26006, the "B" LPI valve, was tested and HV-26046 and 26047, DHR crosstie valves, were opened prior to removing SFV-26005, A-LPI valve, from service.
Appropriate log entries on April 7 indicated testing of SFV-26005, SFV-23645 and returning of the DHR crosstie valves to normal following the breaker inspections. This completed the required redundant component testing prior to removal from service, as well as the testing required following maintenanc.e necessary to assure operability.
The one discrepancy noted was that there was no log entry concerning the testing of SFV-23646 prior to removing SFV-23645 from service. A discussion with the operating personnel and the inspector concluded that the required surveillance, although probably performed, had not been documented in either the Control Room or Shift Supervisors Log.
To prevent or minimize similar occurrences in the future, the Supervisor of Nuclear Operations issued a memo, SO 14-78 dated May 22, 1978, to all Shift Supervisors. This memo informed all Shif t Supervisors of the deficiency and reminded them of the necessity for prompt and complete log entries on all such actions.
