This report is submitted in accordance with Technical Specification 6.9.1 and Regulatory Guide 1.16, Revision 4, Section 2.a. (9) as a written Four-teen (14) Day Report. Your office was notified on' 3 February 1977 of-this event.
While in Mode 4 (Hot Shutdown), two Decay Heat Valves (DHV-4 and DHV-41) on the pressure drop line did not open on Control Center actuation. They were quickly opened by hand.
The cause of this event is believed to be the effect of pressure acting on the gate valve discs. A modification to provide a relief line has been initiated to prevent recurrence. In addition to this, as an interim measure, the torque setting on the valve operator has been increased.
No adverse consequences resulted from this event.
