N ESSEX October 5,1979 Ms. Mary Jo Mattia Chief, Administrative Contract Branch Division of Contracts Office of Administration United States Nuclear Regulatory Commission Washington, DC 20555
Dear Ms. Mattia:
The Essex Corporation has been under contract to the NRC for a human factors evaluation of control room design and operator performance at TMI (Contract Number NRC-04-79-209). This contract is for $96,236.
The purpose of this letter is to request an extension of the final report due date, and to request additional funding based on an enlarged contract scope. We are requesting that the duc date for the draf t report be changed from September 30, 1979 to October 22, 1979. We are further requesting additional funding at the level of $37,814 to cover costs incurred in an expanded effort. A DD-633 is attached to indicate the basis for this cost estimate.
The change in scope of the contract was essentially due to delays in receiving information from NRC which was essential to our effort. Since we were not receiving this information, and since we required it before progressing with the work, we were lef t with no option but to develop the information ourselves. This not only increased the scope of the contract but also resulted in the three week delay of production of the final report.
The specific items of information which were required and which were not provided in a timely manner are as follows:
TMI-2 control panel drawings and functional descriptions.
e Met. Ed. documents describing operator training programs, the training e
received by the operators, operator selection criteria, and operator qualifi-cation records.
Met. Ed. CR and human factors design analyses and reports on special e
studies relating to the CR and human factors considerations.
A list of critical points of action / inaction in the event chronology which e
contributed significantly to the source of the accident.
l in addition, problems and delays were encountered in:
Obtaining a complete set of reactor technology memoranda.
e Obtaining out-of-print stanom:ds Irom ANS prior to 1975.
j o
Developing the full-scale mockup of the control consoles in the absence of e
panel drawings and panel photographs from Met. Ed..

e Evaluating control and display relationships without panel drawings.
o Identifying control and display functions in the absence of control-display e
functional descriptions.
Identifying controls and displays to be evaluated in plants of the same e
vintage in the absence of control-display functional descriptions, Scheduling meetings with operators, especially for purposes of having them e
contribute to the completion of the time lines.
Finally, delays were experienced due to changes in the assignment of a contract technical representative and due to the flooding of NRC facilities which reduced the availability of NRC personnel during the period when the staff was moved.
If you need additional information or clarification, please feel free to contact me.
Sincerely,
/
Richard C. Foote Treasurer RCF:ks
