It is reco== ended that the attached draft IE Bulletin be sent to all licensee's.

Those with OL's for action and those with CP's for infor-mation.

'a'e have recently received eight (8) abnormal occurrences desevibing instances in which safety related instrumentation was in-advertently valved out of service. It is our view that this matter is of sufficient importanc to merit the issuance of a Bulletin.

Descriotion of Circumstances We have received several abnormal occurren'ce reports which identified instances.when safety related instru=entation was valved out of service.

In one instance high dryvell pressure instrumentation isolation valves at containment vessel penetrations were closed.

In another instance an instrument was valved out for maintenance during a shutdown and the reactor was restarted without the instrument being returned to service.

Action Recuested of Licensees In light of these type occurrences you are requested to take the following actions. Upon completion of these actions you are requested to inform this office in writing, within 90 days of the date of this letter, of the status of each ite= listed below.

A copy of your report should be sent to the I&E Assistant Director for Operations and Construction.

Procedural Review 1.

Conduct a review of operating, maintenance, testing and I&C procedures to determine that procedures exist that clearly identify the correct position of all instru=entation valves in safety related uses.

Verify that checkoffs are included in procedures to assure that each valve is properly aligned.

Procedural signeffs (in.cluding dates and ti=es) by personnel performing valving must be used where appropriate.

Conduct periodic review of the system that assures that there are sufficient checks to verify that valving is and remains correct during the conduct of license activities.

'2.

Assure that necessary steps are being taken so that personnel authorized to perform or to maintain the positions of safety related valves are adequately trained to carry out assigned responsibilities.
