Thornburg, Chief Field Support and Enforcement Branch Office of Inspection and Enforcement PROPOSED BULLETIN - INCORRECI POSITIONING OF SAFE 1T RELATED VALVES The subj ect proposed Bulletin, submitted by Region I, has been evaluated against the criteria for issuance of Bulletins. Whereas the occurrences which prompted Region I to propose the Bulletin are of safety significance, for reasons discussed below, I feel the proposed Bulletin does not meet fully the criteria for Bulletin issuance. The current IE Inspection Program emphasizes repeatedly the importance of licensee controls aimed at insuring that syste=s important to safety are returned to operating status following maintenance, repsir, calibration, etc. Specific inspection procedures which address this subject include Nos. 71710B, 717113 and 71712B. Regulatory Guide 1.33 (Appendix A, paragraph A, items 3. and 10.) also cover the need for administrative controls in this area. In short, our inspection program if properly imple=ented, should identify significant weaknesses in individual licensee's progra=s. Shortcomings in licensee's progra=s should be dealt with in our enforcement prpgram on an individual basis. In sum =ary, the subject proposed Bulletin does point cut significant weaknesses in the aMnistrative controls of several facilities in Region I. The IE inspection program emphasizes the need for an effective program of control by licensees, and calls for frequent examination of the licensee's program at each operat:'ng power reactor. Inspection and Enforcement Bulletin 75 March , 1975 INCORRECT POSITIONING OF SAFETY RELATED VALVES Description of Circumstances We have received several abnormal occurrence reports which identified instances when safety related instrumentation was valved out of service. In.one instance high drywell pressure ins.trumentation isolation valves at contain=cr.t vessel penetrations were closed. In another instance an instrument was valved out for maintenance during a shutdown and the reactor was restarted without the instrenent being returned to service. Action Recuested of Licensees In light of these type occurrences you are requested to take the following actions. Upon co=pletion of these actions you are requested to inform this office in writing, within 90 days of the date of this letter, of the status of each item listed below. A copy of your report should be sent to the I&E Assistant Director for' Operations and Construction. Conduct a review of operating, maintenance, testing and I&C procedures to determine that procedures exist that clearly identify the correct position of all instru=entation valves in safety related uses.

Verify that checkoffs are included in procedures to assure that each valve is properly aligned. Procedural signoffs (including dates and times) by personnel performing valving must be used where appropriate. b. Management and OA Controls i 1. Conduct periodic review of the system that assures that there are sufficient checks to verify that valving is and remains correct during the conduct of license activities. 2. Assure that necessary steps are being taken so that personnel l ~ authorized to perform or to maintain the positions of safety L related valves are adequately trained to carry out assigned L responsibilities.
