Dear Mr. Arnold:

I We note that according to the GPU Inter-Office Memorandum, TSG-377, dated August 22,1979 (see enclosure), GPU committed itself to carry out an extensive Quality Assurance and Quality Control Program on the MDHR Systan. In order to assure conspliance with the listed program

objectives, we request a copy of the documentation identified in items 1 through 6 of the enclosure and notification when the welds have been i

finished. A submittal of the hydrostatic and start up test procedures is requested.

i In addition, since the prefabricated piping spool pieces have been found to have marginally acceptable welds we require th have the new fabricated welds to be radiographed where possible and dye penetrant tested where I

they are inaccessible, rh-insion of the above inspections should be justified.

We request the above information as soon as possible so as not to impact the current MDHR syarea installation schedule.
