Date ComponentsSection II Vendor Inspection Branch Summary Inspection on January 2-4, 1980 (99900274/80-01)
Areas Inspected:
Implementation of 10 CFR 50, Appendix B criteria including action on previous inspection findings, implementation of QA Manual / Program, and product qualification documentation.
The inspection involved twenty-four (24) inspector hours on site.
Results:
In the three (3) areas inspected; two (2) deviations and one (1) unresolved item were identified in two (2) areas while no findings were identified in the third area. The findings are described in the following paragraphs:
Deviations:
Implementation of QA Mr.nual/ Program - (1) QC Manual does not delineate in writing the authority and duties of management personnel whose activities affect quality (See Notice of Deviation, Item A); (2)
Contrary to.a contract commitment, no single record storage facility is maintained nor is a complete set of duplicate records maintained.
Also procedures for filing, storage and-retention of nuclear safety related products have not been compiled and implemented (Notice of Deviation, Item B).
Unresolved Items:
Product Qualification Documentation - Qualification documen-tation reports not complete.

Action on Previous Inspection Findings l'.
(Closed) Deviation (Report 77-01):
QC Manual had not been revised as scheduled.
The inspector verified that the QC Manual was revised and reissued as revision 6, November 15, 1979.

(Closed) Deviation (Report 78-01):
Certain suppliers are not re-required to provide quality assurance (QA) programs consistent with pertinent provisions of Appendix "B".
Surveys have been made of suppliers whose products may be used in Class IE products.
Sup-pliers whose QA programs meet Forney supplier QA program require-ments are retained as qualified suppliers.
Others are required to improve their QA programs, otherwise they are not retained as qualified suppliers.

(Closed) Unresolved Item (Report 78-01): QA/QC Manual which had been scheduled for revision and completion by the time of inspec-tion 78-01 had not been completed, but was in the draft stages.
The inspector verified the QA/QC Manual had been revised and re-issued to more accurately describe the current QA practices.
(See item 1, above).

(Closed) Unresolved Item (Report 78-01):
Prototype modules with related test procedures which were being prepared for final quali-fication testing at the time of the 78-01 inspection have been tested and the related test procedures issued.
C.
Implementation of QA Manual / Program 1.
Objectives The objectives of this area of the inspection were to:
a.
Meet the new QA Manager and discuss organizational and program changes made since the last NRC inspection.

Review the revised QA Manual and related quality control procedures.
Verify that QA records storage and retention system and c.
facilities for Class IE products meet applicable standards and requirements.
d.
Verify that training has been provided for personnel whose activities affect quality.
Verify that activities of persons and organizations performing e.
activities affecting the safety-related functions for pro-duction of Class IE products are clearly delineated in writing.
f.
Verify that persons and organizations performing quality assurance functions report to a management level such that the required authority and organizational freedom are indepen-dent from cost and schedules.
g.
Verify that a system of document control has been implemented to control documentation for Class IE products.
h.
Verify that a system for handling non-coi ning materials i
and products has been implemented.
2.
Method of Accomplishment The preceding objectives were accomplished by:
Review of the Quality Control Manual, Revision 6, dated a.
November 15, 1979, which incorporates the quality control procedures and instructions used throughout plant operations.
b.
Review of the QA records, contract files and engineering drawing files and storage facilities.
Review of (1) Quality Control Instruction (QCI) 1.3 c.
Quality Audits and (2) QA audit files which revealed that a QA audit plan was implemented in April 1978 and as of that date approximately forty-nine (49) internal audits have been con-ducted.
d.
Review of (1) QC Manual, Introduction and (2) QA department training records and verified that a training program for technician and production personnel had been implemented and is being maintained current. Also, reviewed records indicat-ing that engineering, purchasing and supervisory personnel had received training concerning 10 CFR 50, Appendix B and related standards.

Review of QC Manual, Section 7.0, Quality Control Procedure (QCI) Nonconforming Material and Corrective Action and verified by inspection of the receiving inspection area and the product final inspection area that a system has been implemented for disposition and handling of non-conforming items.

Comments (1) The current QA Manager reports to the Manager of Opera-tions.
This does not appear to be at a management level such that the required authority and organizational freedom will provide independence from costs and schedules.
(2) The current version of the QC Manual, Revision 6, dated November 15, 1979, has eliminated or does not include pertinent QA program criteria that was previously described in the QA Program Plan which has been removed from the QA documentation program. Revision 6 of the QC Manual appetrs to be more of a procedures / instruction manual than a QA Manual.
D.
Product Qualification Documentation 1.
Objectives The objectives of this area of the inspection were to verify that:
a.
Qualification testing of Class IE generic products had been compiled and analyzed.
b.
Qualification test reports had been reviewed and approved by the customer.
c.
Qualification testing had been completed and test data compiled in agreement with the codes and standards imposed by the customer.

d.
Production units (Solid State Isolation Equipment) that had been shipped for contract No. CP-0494, are of the generic type for which qualification testing had been conducted.
2.
Method of Accomplishment The preceding objectives were accomplished by:
Document search of project contract file No. 0740 (customer a.
P.O. No. CP-0494) which revealed the contract had been partially fulfilled by delivery of the required production tested Solid State Irolation Equipment (SSIE).
Included in the contract files were the following documents compiled by tha Forney Engineering Company indicating the SSIE had been designed and Qualification tested in compliance with codes and standards imposed by the related purchase order and equipment specifi-cation 2323-ES-24 Those documents include:
(1) Quality Control Records for Solid State Isolation Equip-ment Part Nos. 302725-01/-02/~03/-04.

Structural Analysis of the Isolation Equipment Cabinet subjected to Seismic Induced Base Motion.
b.
Discussions with contract / engineering personnel who stated that one (1) additional report is forthcoming to support the Forney qualification data of the SSIE to meet applicable en-vironmental and seismic requirenents.
This report, being compiled by Wyle Laboratories and due for completion in February 1980, will be an analysis of the Forney qualification l
test data for the SSIE.

Unresclved Item Although paragraph 7.0 conclusion of the " Test Report, Qualification, Solid State Isolation Equipment, A-302743-01" states that the SSIE meets the qualification requirements in accordance with the intent of the criteria of IEEE Standards
-324 1974 and -334 1975 the Forney Engineering Company wishes to await compilation of the Wyle Laboratories analysis report of the SSIE qualification test data to substantiate its position on the qualification of the SSIE.
Therefore, this item will remain open pending review of the Wyle Laboratories report during a subsequent inspection.
E.
Exit Interview The inspector met with management representatives denoted in paragraph A above, at the conclusion of the inspection on January 4, 1980.
In this meeting the inspector discussed the scope and findings of the inspection covering the following areas:
1.
Action on Previous Inspection Findings 2.
Implementation of QA Manual / Program 3.
Product Qualification Documentation.
Management acknowledged statements made by the inspector.
