[VD Appendix A NOTICE OF VIOLATION Detroit Edison Company Docket No. 50-341 Based on the results of the NRC inspection conducted on December 18-21, 1979, it appears that certain of your activities were in noncompliance with NRC requirements as noted below.
These items are infractions.
1.
10 CFR 50, Appendix B, Criterion V requires tha;. activities affecting quality shall be accomplished in accordance with documented procedures.
Enrico Fermi QA Manual, QAP No. 9, subparagraph 9.1.5 states in part, " Activities related to the performance of quality assurance functions affect quality and shall be performed in accordance with prescribed procedures."
Contrary to the above, you failed to stop mechanical installation work in accordance with subparagraph 4.2.1.6 of Section No. I of the Wismer and Becker Quality Assurance and Control Manual even though construction supervision did not assure that work operation
- did not proceed past mandatory hold points as specified in subparagraph 3.3 of Section X of the above manual.

10 CFR 50, Appendix B, Criterion XVI requires that conditions adverse to quality be corrected and precluded from repetition.
Enrico Fermi QA Manual, QAP No. 17 subparagraphs 17.1.1 and 17.1.3 state in part, "... quality personnel shall assure the cause of conditions adverse to quality is determined, and take corrective action to preclude repetition."
Contrary to the above, during the past year over 40 instances had been identified where mechanical installation inspection hold points were by passed; you failed to determined the cause and take corrective action to preclude repetition.

10 CFR 50, Appendix B, Criterion X requires that mandatory inspection j
hold points which require witnessing or inspection and beyond which work shall not proceed, be indicated on appropriate documents.
Enrico Fermi QA Manual, QAP No. 11, subparagraph 11.1.6 states in part, "when mandatory inspection hold points beyond which work must not proceed until signed off by Edison or its agent are required they shall be indicated in appropriate documents before work is initiated."
