APPENDIX A NOTICE OF VIOLATION Bgstgn Edisgn Cgmpany 1 Docket No. 50-293 Based on t'e results of an NRC inspection conducted on December 3-7, 1979, it appears that one of your activities was not conducted in full compliance with the Code of Federal Regulations and your Facility License No. OPR-35 as indi-cated below.

This item has been categorized as a deficiency.

Technical Specification 6.9.B.2.b states that a thirty day report be submitted to the NRC whenever conditions lead to operation in a degraded mode permitted by a limiting condition for Operation.

Technical Specification 6.9.8.2.e states that a thirty day report be submitted to the NRC for any changes in corporate or station organization as described in section 6.2.

Technical Specification Appen-dix B, section 1.A, B states that the NRC shall be notified within 30 days when-ever the temperature rise of condenser cooling water between the intake structure and the discharge to the bay exceeds 32*F.

Contrary to the above the following occurrences were not reported to the NRC.

On July 10, 1979 and August 7, 1979 the temperature rise of condenser cool-ing water between the intake structure and the discharge to the bay exceeded 32 F.

On August 7, 1979 the shutdown transformer failed, placing the plant in a degraded mode permitted by Technical Specification 3.9.8.1.

l On July 5, 1979 the HPC1 room smoke detector failed, placing the plant in a degraded mode permitted by Technical Specification 3.12. A.

In August, 1979 the station implemented on organizational change which created the position of Assistant Station Manager.
