NRC Inspection 78-12

Dear Sir:

The inspection conducted by Mr. R. Thomas and J. Curtis on December 4 to 8, 1978 stated that ore -f our activities was not in cotpliance with NRC requirements.

Appeadix A of your letter states:

Cont rary to 10 CFR 20.203(f), at the time of inspection, two vent rig filt er containers located in the West Decay Heat Room of the Auxilary Building, were not labeled to indicate the presence of radioactive ma t e ri al s in excess of the quantities list ed in Appendix C of 10 CFR 20.

The quant ities of radioact ive materials in the cont.siners were sufficient to cause radiation levels of 280 nr/br and 70 mr/hr, respectively, at the surface of the containers.

This is a deficiency.

The following corrective ction has been or will be taken by the dates committed.

Labeling of all containers which actua]Iy contain radioactive material will include " Caution - Radioact E ve Material" or " Danger - Radioactive Material" and the label will be sarked to show workers what dose rate is imanating from from the containers so that the necessary precaution to minimize exposure will be taken.

A general discussion of problems involving radioactive containers and what individuals need to look out for to ensure that these containers are properly labeled will be conducted at routine safety neetings with each group in the maintenance and operating divisions to ensure that personnel are properly made aware of the potential dangers.

These discussions will be concluded by March 31, 1979.

Page 2 February 14, 1979 The following addit ional steps have been undertaken by the llealth Physics organizati:n to reduce the probability of recurrence:

1.

Specific Ilealth Physics personnel have been designated to the inspection and correction of signs, labels, access controls, etc.

2.

Specific ilealth Physics personnel have been designated to carry out quality control ef forts within the HP division for the purpose of carrying out surveillance of signs and labels and to ensure cont rol of Ilealth Physics operations are adequate.

3.

The Plant llealth Physicist has been specifically directed to review on a weekly basis the quality control efforts within the llP division and to document and summari ze the results of his review monthly to management.

4.

Containers which have been purchased for containing radioactive liquids have been surveyed and radioactive warning labels af fixed prior-to their use.

The infornation and instructional ncetings that will be conducted and the additional placement of responsibilities regarding the monitoring and surveying of cont ainers placed upon the lleal th Physics Division should pre-clude the recurrence of the deficiency not ed during t he December 4-8 inspection.
