APPENDIX A
' NOTICE OF VIOLATION Philadelphia Electric Company Docket No. 50-278 Based on the results of an NRC inspection conducted on Octob'er 30-November 3,1978, it appears that one of your activities was not con-ducted in full compliance with the conditions of your NRC Facility License No. DPR-56 as indicated below.
This item is an Infraction.
Technical Specification 6.13.1.b requires in part that for "Each High Radiation Area in which the intensity of radiation is greater than 1000 mrem / hour... locked doors shall be provided to prevent unauthorized entry into such areas...".
Contrary to the above on November 2, 1978 the 3B and 4B FWH door in the Unit 3 Turbine building, elevation 135', a designated high radiation area which contained radiation levels as based on the most recent survey dated October 31, 1978 varying from 12-1200 mrem /hr was found not maintained locked.
