Dear Mr. Keppler:

The following is in response to an inspection conducted by Mr. L. J. Hueter and N. J. Chrissotimos on December 11-14, 1979 of activities at the Quad Cities Nuclear Power Station, Unit 2. Reference (a) indicateo thuc cartain activities appeared to be in noncompliance with NRC requirements. Attachment A to this letter contains Commonwealth Edison Company's response to the identified items of noncompliance.

  • _2 CORRECTIVE ACTION TAKEN AND RESULTS ACHIEVED At the time of the NRC inspection, both individuals involved were interviewed to find out the details of how they reelved their daily exposures in excess of 50 mrem. These individuals were re-instructed on the requirements for obtaining an SWP or H. P. surveillance when they expect to receive daily exposures in excess of 50 milllrems.

CORRECTIVE ACTION TO BE TAKEN TO AVOID FURTHER NON-COMPLIANCE The annual retraining program for Station employes will be reviewed, and any necessary changes made, so that material pertaining to daily exposures in excess of 50 milllrens per day is clarified and emphasized.

The Station policy of allowing Senior Licensed Operators to enter high radiation areas without a timekeeper will be reviewed and specific guidelines for exposure control will be determined.

DATE WHEN FULL COMPLIANCE WILL BE ACHIEVED The review of the annual retraining program, and any changes, will be completed by June 1, 1980. The guidelines for SRO's entering high radiation areas will be completed by May 1, 1980.

3. Contrary to Technical Specification 3.8.D.l., the unplanned radioactive liquid release which began about 10 a.m. on December 9, 1979, was not

-continuously monitored.

CORRECTIVE ACTION TAKEN AND RESULTS ACHIEVED The immediate corrective action taken was to terminate the release from the 2B RHR Service Water piping. On December 10,'1979, the effluent from the Waste Water Treatment Plant was terminated, and the storm sewer system was later analyzed and discharged as radioactive liquid releases in accordance with Station procedures.

CORRECTIVE ACTION TO BE TAKEN TO AVOID FURTHER NON-COMPLIANCE The composite samplers on the spray canal blowdown line and on the South diffuser pipe will normally be operated on a continuous basis, so that any unplanned releases via either release path will be monitored.

COMMONWEALTH EDISON COMPANY Attachment A Notice of Violation Items on Non-Compliance identified in Appendix A to the NRC letter dated January 26, 1980, are responded to in the following paragraphs.

1. Contrary to Technical Specification 6.2.A., on December 9, 1979, Temporary Procedure 1261 " Draining Unit 2 RHR Service Water Pipirig" was not adhered to in three instances, resulting in an unplanned radioactive liquid affluent release to the Mississippi River.

CORRECTIVE ACTION TAKEN AND RESULTS ACHIEVED Discussions were held with the personnel involved with the draining operation. These discussions addressed the procedure itself and the specific steps that were not followed.

Appropriate personnel were reprimandad, and disciplinary action has been taken against certain individuals. This event has been discussed in the Station Department Head meeting, as well as in individual department meetings. Strict adherence to procedures has been stressed to all departmental areas.

CORRECTIVE ACTION TO BE TAKEN TO AVOID FURTHER NON-COMPLIANCE In the future, when procedures are developed to accomplish draining of systems to outside the Station, steps will be added to provide for sampling to beperformed prior to re-initiating draining operations after valve line-up changes.

This will result in detecting possible contamination prior to release, and prevent uncontrolled releases to the environment.

DATE WHEN FULL COMPLIANCE WILL BE ACHIEVED Full compliance is achieved at this time'.

2. Contrary to Technical Specification 6.2.B and procedure QRP 100-1 paragraph F.16.A.2, two workers received greater than 50 mrem dose on December o, 1979, without authorization provided by an SWP or continuous H. P. surveillance.
