9 Appendix A NOTICE OF VI0I/ TION Commonwealth Edison Company Docket No. 50-454; 50-455 Based on the results of the NRC Investigation conducted on July 8, August 27-28, September 11-12, and November 5, 1979, it appears that certain of your activities were in noncompliance with NRC requirements as noted below. These items are infractions.
1.
10 CFR 50, Appendix B, Criterion VI states, in part " Measures shall be established to control the issuance of documents.
. including changes thereto.
Commonwealth Edison Company Topical Report CE-1-A, Revision 9, Section 6, states in part, "A document control system will be used to assure that documents
... are reviewed for adequacy and approved for release by authorized personnel changes to these documents will be handled similarly...."
Contrary to the above:
a.
Incorrect cable connectors had been installed and identified on noncontrolled Observation Memo No. 53, dated April 25, 1979, rather than in accordance with the established document control system.
b.
Concrete expansion anchors had been identified as nonconforming with Standard BY/BR-CEA, which had been issued after the instal-lation of the anchors. However, the nonconforming situations had not been documented in accordance with the established docu-ment control system.

10 CFR 50, Appendix B, Criterion X states, in part "A program for inspection of activities affecting quality shall be established and executed.... to verify conformance with the documented instruc-tions, procedures, and drawings Commonwealth Edison Company Topical Report CE-1-A, Revision 9, Section 10, states in part, " Inspection Programs will be establish-ed to provide assurance that the quality control surveillance, in-spections.... are performed...."
Contrary to the above, on November 5, 1979, an adequate program had not been established to assure that concrete expansion anchor plumbness, edge and spacing, and exposed length were inspected by all site contractors.
