Dear Mr. Seidle:
On December 14, 1979, Messrs. Bill Crossman and Ray Hall of your office were informed of a situation at Wolf Creek which could possibly have a significant impact on safety.
The situation involves a NAMCO Snaplock limit switch EA-180-31302 (installed on one of the main steam isolation valves) which was received on site with its actuation arm bent and cracked (arm eventually fell off).
The particular "open positions" application of this limit switch involves a redundant switch; thus, the safety impact in this case would be minimal.
However, an investigation is being made to detennine if this is an isolated case or a generic problem. The supplier of the valve to Wo f Creek has been contacted and has reques'. d that the subvendor who 1 tpplied the switch be involved in the inspection and evaluation.
If the results of the investigation indicate a generic problem, a report will be issued in accordance with 10CFR50.55(e) requirements.
If it is determined that the failure rep-resents an isolated case, we will so advise you.
An update on the status of our investigation will be forthcoming within sixty (60) days if the final results are not Pnown by that time.
