FINAL REPORT - CONCRETE TESTING SERVICES The following report is offered in fulfillment of the recuirements of 10CFR 50.55(e) (3).

Description of the Deficiency Our letter of December 3, 1979, described to you a defi-ciency in the Quality Control of concrete testing services at Southwestern Laboratories. We have concluded our investigation of the matter and make the following final report.

Analysis of Safety Implications The tests performed by Southwestern Laboratories were for qualification of concrete materials, i.e. cement, mix water, and fine and coarse aggregates. Errors in this testing program could have led to a situation where nonconforming and possibly inadequate structural concrete could have been placed in safety-related structures.

Based upon the report submitted to you by our letter of December 6, 1979, the safety-related concrete placed prior to the stop work of November 2, 1979 (these included the mud mats for the reactor building, control building, standby cooling towers, auxiliary building, and the base mats for some pump sumps) is acceptable.

Corrective Action The testing laboratory (Southwestern Laboratories) that was originally found deficient in the Quality Control of con-crete testing has been relieved of its testing responsibilities.

As of October 25, 1979, all testing is being performed by Pittsburgh Testing Laboratory. An in-process surveillance in-spection was performed on Pittsburgh Testing on November 18, 1979, and the results were satisfactory.

The job materials were retested, using Pittsburgh Testing Laboratory. A complete report on the details of this testing program was submitted to you in our letter of December 6, 1979.

Further, we have instituted increased frequency of all testing (i.e., every 700 tons instead of the originally spec-ified 1,200 tons) to more closely monitor the cement used on this j ob . The testing frequencies for alkali content comply with condition 3 of your letter of December 13, 1979.

This concludes our report on the investigation and satis-factory corrective action taken in resolution of the deficiency.
