Cooper Energy Services En-Tronic Controls Docket No. 99900373/79-01 NOTICE OF DEVIATION Based on the results of in NRC inspection conducted on July 23-26, 1979, it appears that certain if your activities were not conducted in accordance with E requirements.

Criterica V of Appendix B to 10 CFR 50 states:

" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.

Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for det. -mining that important activities have been satisfactorily accomplished." Deviations from these requirements are as follows:

A.

Paragraph 3.6 of the Quality Assurance t!anual, Revision 2, dated November 30, 1978, states in part, "The Project Engineer prepares a basic test program for each project which contains the individual system tests, the special tests required for the project and t.oe witness and hold points."

Engineering Procedure No. EE-163, Revision 0, dated October 20, 1977, states in part, "The En-Tronic Project Engineer will issue the test requirements for the particular project before manufacturing starts at En-Tronic or major subsuppliers."

Contrary to the above, Project Engineers had not prepared a basic test program or issued the test requirements before manufacturing had started on the Arizona and Houston projects.

Paragraph 5.2 of the Quality Assurance flanual, Revision 2, dated November 30, 1978 states, "The test instructions / check list are made for each unit and are on file.

Copies are available.

The instructions provide criteria for acceptance and compliance with thc drawings, instruction and procedures.

These completed test instructions are approved by the Quality Assurance t!anager and filed as a permanent record."

Contrary to the above, test instructions / checklist, dated June 11, 1979, for unit number 59-7183-88 did not provide criteria for acceptance of nicroampere and megohm measurements or speed monitor points.

C.

Paragraph 6.1 of the Quality Assurance ?!anual, Revision 2, dated November 30, 1978, states "tieasures have been provided to assure that design documents, including changes, are prepared, reviewed for adequacy, and approved for release by authorized personnel within the Engineering department. The Chief Designer reviews and approves all documents before released for distribution."

2 Paragraph 6.2 of the Quality Assurance Manual, Revision 2, dated November 30, 1978, states in part, "Each station is responsible for filing new issue prints and destroying prints which sie being replaced by new issue."

Contrary to the above:

1.

Bill of Material No. G5-262-1017, Revision 4, dated July 2, 1979, had act been reviewed and approved; i.e., the " Approved" space exhibited no initials or signature.

2.

Bill of Material No. G5-262-1009, Revisions 0 and 2, dated December 15, 1977 and July 7, 1978 respectively, had not been removed from the Planner's file upon receipt of Revision 3, dated April 18, 1979; i.e.,

the file contained these three (3) revisions of the Bill of Material.

D.

Engineering Procedure No. EE-132, Revision 1, dated November 1,1976, states in part, "To assure proper completion or action on the shop-initiated changes, these notifications Form 35 will be recorded by the Chief Designer and are to be initialed by the Project Engineer or Project Draf tsman af ter proper action has been taken, that is, master prints have been marked, drawings have been revised or requested action has been taken and returned to the Chief Designer."

Contrary to the above, notification serial numbers 2007, dated May 30, 1979; 2009, dated June 1, 1979; 2013, dated June 6, 1979; and 2014, dated June 10, 1979; had not been initialed despite the master prints having been marked.

In addition to the above, the following deviation on QA program content was identified:

Criterion VI of Appendix B to 10 CFR 50 states in part, " Measures shall be established to control the issuance of documents, such as instructions, procedures, and drawings, including changes thereto, which prescribe all activities a f fecting quality.

These measures shall assure that documents, including changes, are reviewed for adequacy and approved for release by authorized personnet.

Contrary to the above, with the exception of Engineering documents, measures are not establi.hed by the QA program to provide for the review and approva) of documents which prescribe activities affecting quality.

Specific examples are Quality Assurance Procedures and Manufacturing Procedures.
