As a result of our review, we find that additional information is needed.
Specifically; 1.
Pleare provide a description of steps.that have been or will be taken to prepare a basic test program ac.d issue test re-quiremerts for the Arizona and Houston projects.
This refers to Item A, of the Notice of Deviation, 2.
Please provide the effective date of your preventive measures and a clarification of " periodically," This refers to Item C, of the Notice of Deviation.

Please provide a description of steps that have been or will be taken to initial the identified Form 35s; also provide the effect-ive date.
Additionally, please provide che effective date of your preventive measures and a clarification of " periodically," This refers to Item D. of the Notice of Deviation, 4.
Please provide a description of steps that have been or will be taken to establish measures that provide for review and approval of documents which prescribe activities affecting quality. Also, please provide preventive measures related to non-establishment of measures.
Please provide effective dates of your corrective action and preventive measurea. This refers to Item E, of the Notice of Deviation.
