Appendix A NOTICE OF VIOLATION College of Saint Teresa License No. 22-03475-03 Based on the inspection conducted on October 24, 1980, it appears that certain of your activities were in noncompliance with NRC requitements, as noted below, ltems 1 and 2 are infractions, and items 3 and 4 are deficiencies.
1.
Paragraph 10 CF; 30.3, Title 10 of the Code of Federal Regulations, Part 30, state 1, in part, no person shall own, possess, or use, by-product material except as authorized in a specific or general license issued pursuant to the regulations.
Contrary to this requirement, on the date of this inspection, the licensee possessed a vial containing strontium-90, byproduct material not authorized by this license.

License Condition No. 14 of SRC Byproduct Material License No. 22-03475-03, states, in part, licensed material shall be possessed and used in accordance with statements, representations, and procedures contained in application dated March 15, 1979.
Item 11 of this application specifies that portable monitors and the Classmaster Rate Meters and Scalers are checked at the beginning of each academic year (September) by the Radiation Detection Officer.
Contrary to this requirenent, as of the date of this inspection, the portable survey instruments have not been checked ar. specified since the Spring term of 1979.
3, Paragraph 10 CFR 30.51 (a), Title 10 of the Code of Federal Regulations, Part 30, states, in part, each person who receives byproduct material pursuant to a l? cense issued pursuant to the regulations shall keep records showing the receipt, transfer, and disposal of such material.
Contrary to this requirement, as of the date of this inspection, no receipt records were available for iodine-131 received during the past two years nor were records available for the strontium-90 in storage.

Paragraphs 10 CFR 19.ll(a) or (b) and (c), Title 10 of the Code of Federal Regulations, Part 19, require specific documents be posted.
Contrary to these requirements, as of the date of this inspection:
A.
A copy of the license, Parts 19 and 20 of the regulations and the operating procedures applicable to the licensed activities were not posted nor was a notice posted of where these docu-ments may be examined as required by 10 CFR 19.11(a) or (b).
B.
A copy of Form NRC-3, " Notice to Employees" was not posted as required by 10 CFR 19.11(c).
