SUBJECT:
LEAKING SRONTIUM 90 SOURCES Enclosed is a memorandum from Region III dated October 3,1980, describing leakin:
Sr-90 sources received by Victoreen, Inc., Cleveland, Ohio from ICN, Irvine, California. Of 54 sources received by Victoreen, none were found to be leaking originally. However, later tests showed eleven sources were leaking as indicated on the attachment to the enclosure. These leaktng sources and the remainder of the 54 were subsequently transferred back to ICN.
Because of so many leaking sources that were found, a generic problem may exist in the manufacture of the sources by ICN. Therefore, we request that California be contacted to followup on this matter. If California finds that a generic problem exists, Region III would like to have ICH supply a list to California of all licensees in Region III who have received this type of source. The list need go back no further than one year'. The list should be forwarded directly to Region III. Based on the followup findings by Region III, it may be necessary to issue a circular to all ifcensees or have ICN recall the sources, the latter being the best course of action in our opir.fon.
Please advise J. R. Metzger of my staff of California's findings following thhfr conclusion of an investigation.
