APPENDIX A NOTICE OF VIOLATION Baltimore Gas and Electric Company Docket No. 50-317 Based on the results of an NRC inspection conducted on October 2-5, 1979, it appears that one of your activities was not conducted in full compliance with the conditions of your license DPR-53 as indicated below.

This item is a deficiency.

10 CFR 55, Appendix A, Section 3.c, states that, "The requalification pro-gram shall include on-the-job training so that:

...c.

Each licensed operator and senior operator is cognizant of facility design changes...."

10 CFR 55 Appendix A section 5 states that "a.

Records of the requaliff-cation program shall be maintained for a period of two years from the date of the recorded event..."

Calvert Cliffs Instruction 604B " Licensed Operator Training" dated April 11, 1978 section IV.E.1 states that:

"A minimum of forty hours of formal requalification lectures will be administered during each annual period.

The lectures will be comprised of the following..." and section IV.E.,

i.e., " changes to plant design..." and section IV.E.2 "the lectures must be attended by all licensed personnel or a satisfactory make up assignment completed."

Section IV.E.5 states: "The portion of the requalification program lecture series which deals with design changes...will be conducted in a special manner....

The training coordinator will perform one or a combination of the following methods for prompt dissemination of a description of the FCR to licensed personnel.

a)

Scheduling a special lecture to discuss the FCR.

b)

Circulation of a written description of the FCR and its effect on plant operation.

c)

Circulation of a brief description of tne FCR with the detailed dis-cussion to take place during regulatory scheduled requalification lecture.

Contrary to the above requirements on October 5, 1979 at the conclu-sior. of this inspection:

1.

For the regularly scheduled design change lecture conducted on June 7,14, 21 and 28,1978 only four licensed operators attanded and the licensee had no documentation to show that all remaining license holders who were actively exercising their licenses had completed make up assignments.

2.

There was no documentation to support that the active licensed and senior licensed operators had made facility design reviews for the year 1978.
