APPENDIX A NOTICE OF VIOLATION Philadelphia Electric Company Docket No. 50-278 Based on the results of an NRC inspection conducted on October 1 -
October 31, 1979, it appears that one of your activities was not conducted in full compliance with conditions of your NRC Facility License Nos. DPR-44 and DPR-56 as indicated below.
This item is a deficiency.
Technical Specification 3.10. A.1 states in part, "The reactor mode switch shall be locked in the " Refuel" position during core al terations...... ".
Contrary to the above, on October 9, 1979, the Unit 3 mode switch was found in refuel with the key in place and core alterations had occurred prior to key removal.
