APPENDIX A NOTICE OF VIOLATION United Nuclear Corporation Docket No.70-820 UNC Recovery Systems.
This refers to the investigation conducted by representatives of the Region I (Philadelphia) office at United Nuclear Corporation, UNC Recovery Systems, Wood River Junction, Rhode Island, of activities authorized by NRC License No. SNM-777.
During this investigation, conducted June 25 and 26,1979, the following apparent item of noncompliance was identified.

10 CFR 70.54 requires, in part, that each licensee who transfers special nuclear material shall ccmplete and distribute Nuclear Material Transaction Report on Form DOE /NRC-741, in accordance with the printed instructions for completing this form. These instructions included in the document entitled
" Nuclear Material Transaction Report Instructions (Forms DOE /NRC-741 cnd 741 A)," dated January 1,1979, states, in part, that, "each shipper of reportable quantities of special nuclear material... must dispatch Form DOE /NRC-741 on the same day the material is shipped."
Contrary to the above, a shipment of reportable special nuclear material was shipped to Union Carbide Corporation on June 12, 1979, but the required Form 00E/NRC-741 was intentionally not completed and dispatched until June 19, 1979.
This is an infraction.
