Appendix A NOTICE OF VIOLATION Pharmaco Nuclear, Incorporated License No. 12-17910-01MD Based on the inspection conducted on September 25 and Octob;.10, 1979, s

it appears that certain of your activities were in noncompliance with NRC requirements, as noted belci.

Items 1 through 3 are infractions.

1.

10 CFR 20.201(b) requires you to make such surveys (evaluations) as may be necessary for you to comply with all sections of part 20.

Contrary to this requirement, during the period December 25, 1978, through January 25, 1979, and the period June 24, 1979, through July 24, 1979, you failed to make such surveys as were necessary to assure compliance with that part of 10 CFR 20.101 that limits the radiation exposure to the extremeties of individuals.

Specifically, you failed to evaluate the radiation dose to the hands and fingers of persormel who use radioisotopes.

Condition 16 of your license requires that licensed material be possessed and used in accordance with statements, representations, and procedures contained in your application dated November 15, 1977.

Contrary to Attachment No. 7, Paragraph 2 of this application you failed to make monthly thyroid readings on a person using iodine-131 during the last calendar quarter of 1978 and the first quarter of 1979.

Condition 16 of your license requires that licensed material be possessed and used in accordance with statements, representations, and procedures contained in application dated November 11, 1977.

Contrary to Attachment No. 9, Paragraph 2 of this application you failed to make weekly laboratory surveys during the period May 14 to May 19, 1979, and June 16 to July 3, 1979.
