Maryland 19-02891-05 Based on the results of an NRC inspection conducted on January 8-9, 1979, it appears that one of your activities was not conducted in full compliance with the conditions of your license as indicated below:
Condition 21 of your license requires that licensed material be possessed and used in accordance with the statements, representa-tions, and procedures contained in your letter dated August 3, 1977.
Item g of this letter requires that patient doses containing radioactive material be prepared and administered using lead syringe shields.
Contrary to this requirement, on the day of the inspection, January 9,1979, personnel preparing radiopharmaceuticals failed to use syringe shields in the preparation of patient doses and radiopharmaceuticals prepared from kits.
This is an infraction.
